It has come to my attention that this permit has changed hands from NFP Holdings, LLC to LINCO Partnership. This
additional information request is set to address this ownership change, as well as other issues. I have included the
formal additional information request in this email, along with a few other documents that have already been partially
filled out that you will need to execute. I'm also sending a physical copy to the mailing address that the regional
inspector gave me which is 5200 Old Harding Road, Nashville TN 37064. It's addressed to Mr. Keister, but I don't have a
mailing address for him specifically. If this address is incorrect, please tell me and I can resend it.
Please address all of the items of this request before the end of the day on December 19, 2018. Don't hesitate to
