The Wilmington Regional Office received a Stormwater Management Permit Renewal Application for
Still Meadow Village on August 13, 2007. A preliminary review of that information has determined that
the application is not complete. Only the following checked information is needed to continue the
stormwater review:
❑ Permit Renewal Application. Original signatures must be on the permit renewal application. The
application received by this office appears to be a copy.
❑ Operation and Maintenance Agreement. Original signatures must be on the operation and
maintenance agreement. The operation and maintenance agreement received by this office
appears to be a copy.
❑ A signed Operation and Maintenance Agreement. An Operation and Maintenance Agreement
Form is attached for your convenience.
® Name/Ownership Change Form. A Name/Ownership Change Form is attached for your
convenience.
® Designer's Certification. This office has not received a Designer's Certification for the
Stormwater Management System associated with your facility. Systems that are certified on or
after the date of this letter will need to have the original signature of the certifying person. A
Designer's Certification Form is attached for your convenience.
Please note that this request for additional information is in response to a preliminary review. The
requested information should be received by this Office prior to December 10, 2007. If you need
additional time to submit the information, please mail or fax your request for a time extension to the
Division at the address and fax number at the bottom of this letter. The request must indicate the date by
which you expect to submit the required information.
Please reference the State assigned project number on all correspondence. Any original documents that
need to be revised are enclosed. All original documents must be returned or new originals must be
provided. Copies are not acceptable.

Dear Mr. Beck,
Pursuant your letter of May 23, 2007 I am providing the following information in efforts of
renewing the storm water management permit. I am also including the requested check for the
permit renewal. It is my understanding that the system certification was provided previously by
the Design Engineer Joseph Hill.
We hope that this will suffice as the information necessary to process the requested permit and

The Division of Water Quality issued a Coastal Stormwater Management Permit, Number SW8970726
to South College Associates LP for a High Density stormwater pond to serve Still Meadow Village
Apartments Phase 1 on 12/2/1997. This permit will expire on 12/2/2007. Section .1003(h) of 15 A
NCAC 2H .1000 (the stormwater rules) requires that applications for permit renewals shall be submitted
180 days prior to the expiration of a permit and must be accompanied by a processing fee, which is
currently set at $420.00. If this is still an active project please complete and submit the enclosed
renewal application in a timely manner. If this project has not been constructed and a permit is no
longer needed, please submit a request to have the permit rescinded. If you have sold the project, or
are no longer the permittee, please provide the name, mailing address and phone number of the
person or entity that is now responsible for this permit. Enclosed is a form for change of ownership,
which should be completed and submitted if the property has changed hands.
Your permit requires that upon completion of construction and prior to operation of the permitted
treatment units a certification of completion be submitted to the Division from an appropriate designer
for the system installed. This is to certify that the permitted facility has been installed in accordance with
the permit, the approved plans, specifications and supporting documentation. Please include a copy of
the certification with your permit renewal request and processing fee. Enclosed is a copy of a sample
certification. Also enclosed is a new Operation and Maintenance agreement that should be completed
and submitted along with your renewal application.
You should be aware that failure to provide the Designer's Certification and the operation of a
stormwater treatment facility without a valid permit, are violations of NC General Statute 143-215.1 and
may result in appropriate enforcement action including the assessment of civil penalties of up to
$10,000 per day.

permit transfer for Still Meadow Village. I have forwarded a new blank copy of the required forms to Mr.
Robinson Everette for his signature. He will forward the signed document to you for your signature. The
original document was signed by Mr. Buchanan in the name of South College Associates, LLC, which is no
longer the permittee, and does not have the authority to request a transfer of the permit.
Additionally, please sign the enclosed document, which outlines the Operation and Maintenance
requirements for the system, and return the originals of both the signed transfer form and the O & M form
to me at the address below.

H. REQUIRED ITEMS CHECKLIST
The following checklist outlines design requirements per the Stormwater Best Management Practices anual
m (N.C. Department of Environment, Health and Natural Resources, November 1995) and Administrative Code
Section: 15 A NCAC 2H .1008.

b. The basin length to width ratio is greater than 3:1.
c. The basin side slopes above the permanent pool are no steeper than 3:1.
d. A submerged and vegetated perimeter shelf at less than 6:1 is provided (show detail).
e. Vegetation above the permanent pool elevation is specified.
f. An emergency drain is provided to empty the basin.
g: The permanent pool depth is between 3 and 6 feet (required minimum of 3 feet).
h. The temporary pool draws down in 2 to 5 days.

j. The forebay volume is approximately equal to 20% of the total basin volume.

Footnotes:
1 when using the Division SA/DA tables, the correct SA/DA ratio for permanent pool sizing should be computed based upon the
actual impervious % and permanent pool depth. Linear interpolation should be emploved to determine the correct value for non-
standard table entries.
The requirement for a vesetative filter may be waived if the wet detention basin is designed to meet 90% TSS removal.
3 The legally responsible party should not be a homeowners association unless more than 50% of the lots have been sold and a
resident of the subdivision has been named the president.
III. WET DETENTION BASIN OPERATION AND KAEiTENANCE AGREEMENT
The wet detention basin system is defined as the wet detention basin, forebay, non-integrated pretreatment
systems (pretreatment other than forebays), and the vegetated filter if one is provided.

Maintenance activities shall be performs as follows:
I. After every significant runoff producing rainfall event and at least monthly:

a; Inspect the wet detention basin system for sediment accumulation, erosion. trash accumulation, grass
cover, and general condition.
b. Check and clear the orifice of any obstructions such that drawdown of the temporary pool occurs within
2 to 5 days.
2. Repair eroded areas immediately, re -seed as necessary to maintain good vegetative cover, mow vegetative
cover to maintain a maximum height of six inches, and remove trash as needed.
3. Inspect and repair the collection system (i.e. catch basins, piping, swales, riprap, etc.) quarterly to maintain
proper functioning.
4. Remove accumulated sediment from the wet detention basin system semi-annually or when depth is
reduced to 75% of the original design depth. Removed sediment shall be disposed of in an appropriate
manner and shall not be handled in a manner that will adversely impact water quality (i.e. stockpiling near a
wet detention basin or stream, etc.).

5. Remove cattails and other indigenous wetland plants when they cover 75% of the basin surface. These
plants shall be encouraged to grow along the basin perimeter.
6. If the basin must be drained for an emergency or to perform maintenance, the flushing of sediment through
the emergency drain shall be minimized to the maximum extent practical.
7. All components of the wet detention basin system shall be maintained in good working order.
