The Permittce shall provide Reuse Water public Education Packages to each new and existing customer of the Kinnakeet Shores Subdivision Wastewater Treatment Plant. TY. ()PE1tA'1C]<ON AND MAQUENANCE MOMEM9M 1. The facilides shall be properly maintained and operatcd'at all times. 2. Upon classification of the facility by the Water Pollution Control System Operators Certification Commission (WPCSOCC), the Permittee shall employ a certified wastewater treatment plant operator to be in responsible charge (ORGY of the wastewater ant facilities. The operator must hold a certificate of the type and grade at least equivalent to or greater than the cla4s3fic ati.on assigned to the wastewater treatment facilities by the WPCSOCC. The P=yAttee must also employ a certified back- up operator of the appropriate type and grade to comply with the conditions of 15A NCAC 8(} .0202. The ORC of the facility, must visit each Class I facility at least weekly and each Class II, III, and IV facility at least da lly, excluding weekends and Holidays, and must properly manage and document daily operation and maintenance of the facility and must comply With sll other conditions of 15A NCAC 80,0201 Otace the facility is classified, the Permittee must submit a letter to the WPCSOCC which designates the operator in responsible charge within thirty days after the wastewater t vaun nt facilities are SO`go complete. 3. A. suitable, year-round vegetative cover shall be maintained on the irrigation areas. 4. Irrigation shall not be performed during inclement weather or when the ground is in a condition that will cause runoff. 5, Adequate measures shall be taken to pmvcnt wastewater nmoff from the spmy fields. 6. The facilities shall be effectively maintained and operated as a non -discharge system to prevant the discharge of any wastewater resulting from the operation of this facility. 7. The ap lication rate for the storage/infiltration 'pond shall not exceed a monthly average of 1.49 GPD/ft$ S. The application of chemicals to the storageHnffitration pond is expressly prohibited. 9. The application rate shall not exceed a cumulative' loading of 161.2 inches over any twelve (12) month period at an instantaneous application rate not to exceed, 0.3 inches per hour in irrigation zones Al through Al2. 10. No type of wastewater other than that from the Kinnakeet Shores Subdivision Wastewater Treatment Plant shaA be sprayed onto the irrigation area. 11. No type of wastewater other than that from the Khmakeet Shores Subdivision Wastewater Treatment Plant shall -be disposed in the storagermfyltration pond. 1'A waste level gauge, to'monitor waste levels in the storagelinfltrati(m pond, shall be installed.. This gaugeshall have readily visible pelmaneut tnarldngs indicating the nw-imum liquid level at the top of the temporary liquid storage volunne, minimum liquid level at the bottom of the temporary liquid storage volamac, and top of the darn elevations. Caution must be taken not to damage the integrity of the liner when installing the gauge. 13. Any monitoring (including groundwater, surface water, soil or plant time analyses) deemed necessary by the Divisibn to insure surface and ground water protection will be established and an acceptable sampling reporting schedule shall be followed. 5. Adequate records shall be maintained by the Permittee tracking the amount of wastewater disposed. These records shall Include, but are not ne-awatily limited to the following information: a- date and time of irrigation, b, volume of wastewater irrigated, c. zone irrigated, d. length of time zone is irrigated, e. continuous weekly, monthly, and year-to-date hydraulic (inches/acre) loadings for each zone, f. weather cmulitions,•and g_ maintenance of cover crops. h, volume of wastewater infiltrated in storagelrofiltration pond A flow meter shall be installed on the irrigation lime to determine volume of wastewater irrigated. The monthly average infiltration flow rate in the storage/inf ltration pond shall be determined by subtracdmg ruse irrigation flow from the flow into the pond. 6. Three (3) copies of all opeittion and disposal records [as specMed in Condition 111(5)) on Form �-' NDAR-I and three (3) copies of all effluent motoring data [as specified in Conditions III(1), M(2), Mpa) and lII(3b)] on Form NDMR-1 shall.be submitted on or bcf= the last day of the following month. Carolina 27699-1617 7. A record shall be maintained of all residuals removed from this facility. This record shall include the name of the hauler, permit authorizing the disposal or a letter from a municipality agreeing to accept the residuals, date the residuals were hauled., and volume of residuals removed. 8. A maintenance log shall be maintained at this facility including but not limited to the following items: a. Daily sampling results iucludin.g residual chlorine, settleable matter, and dissolved oxygen in the aeration basin and at the clarifier weir. b. Visual observations of the plant and plant site. c. Record of preventative maintenance (changing of filters, adjusting belt tensions, alarms testing, diffuser inspections and cleanings, etc.). d. Date of calibration of flow measurement device. e. A representative annual soils analysis (Standard Soil Fertility Analysis) shall be conducted on Irrigation Zones Al through Al2 and the results shall be nmintained on file by the Permittee for a nft num of five years. Solt Fertility Analysis shall include, but is not necessarily, limited to, the following parameters: �- Aridity Manganese Potassium Calcium Percent Hurmic Matter Sodium Copper PH Zinc Magnesium Exchangeable Sodium Perc=tagt Phosphorus Cation Exebange Capacity Base Saturation (by calculation) 10_ Notacompliance Nodfiication: The Permittee shall report by telephone to the Washingtoa Regional Office, telephone number (252) •946-6481, as soon as possible, but in no case more than 24 hours or on the next working day following the occurrence or first knowledge of the occurrence of any of the following: a. Any occurrence at the wastewater treatment facility which results in the treatment of significant amounts of wastes which are abnormal in quantity or characteristic, such as the dumping of the contents of a sludge digester, the known passage of a slug of hazardous substance through the facility; or any other unusual ch%mn Stances. b. Any process unit failure, due to known or unknown reasons, that render the facility incapable -of adequate wastewater treatment such as mechanical or electrical failures of pumps, aerators,' - compressors, W. c. Any failure of a pumping station, sewer line, or treatment facility resulting in a by-pass directly to receiving waters without treatment of all or any portion of the influent to such station or facility. d .may tirme that self -monitoring information indicates that the facility has gone out of compliance with its permit limitations. Persons reporting such omurre ccs by telephone shall also file a written report in letter for within five (5) days following first knowledge of the occurrence. As long as the rotary distributor disposal system is in ,services, monitor wells MW-5, MW-6, MW-7, MW-8 and MW-9 (depicted on the attached maps) shall be sampled every February, June and - September for the following parameters: Water Level pH Chloride Total Dissolved Solids ('MS) Fecal. Qoliforms Total Organic Compounds (TOC) Nitrate (NO3) A unonia Nitrogen Volatile Organic Compounds - In November only using one of the following - (A) Standard Method 6234D. PQL at 0.5 ug/L or less (E) Standard Method 6210D, PQL at 0.5 ug/L or less (C) EPA Method 8021, Low Concentration, PQL at 0.5 ug/L or less (D) EPA Method 8260, Low Concentration, PQL at 0.5 ug/L or less (E) Anoth= method with prior approval by the Groundwater Section Chlef Any method used must meet the following qualifications: (1) A laboratory must be DWQ certified to run any method used. (2) The method used must, at a minimum, include all the constituents listed in Table VIII of Standard Method 6230D. ti (3) 'The method used must prrrvided a PQL at 0.5 ug/L or less which must be supported by laboratory proficiency studies as required by the DWQ Laboratory.Certification Unit. Any constituents detected above the MDL but btlow the PQL at 0.5 uglL must be qualified (estimated) and reported. If any volatile organic compounds are detected by the methods listed, then the Washington Regional Office Groundwater Supervisor, telephone number (252) 946-6481, =st be contacted immediately for father instructio�as regarding any additional follow --up analyses required. The results of all initial and follow-up analysts roust be submitted simultaneously. The measurement of water levels must be made prior to sampling for the remaining parameten. The depth .to water in each well shall be mrasured from, the surveyed point on the top of the casing. The measuring points (top of well casing) of all mo-altoxing wells shall be surveyed to provide the relative elevation of the,measuring point for each mQnitoring well. If TOC concentrations great= than l0.mg/1 are detected in any downp4cut monitoring well, additional sampling and analysis'm= be conducted to identify the individual constituents comprising this TOC concentration. If the TOC conc =tration as measured in the,backgroand monitor well exceeds 10 mg/l, this concentration wiIl be taken to represent the naturally occur ing TOC concentration- A»y exceedances of this natmally occurring TOC concentration in the downgradient wells shall be subject to the additional sampling and analysis as described above. The Operational Agreement between the Pernr►ittee and the Environmental Management Commission Is incorporated berein by reference and shall be a condition of this permit. NoncomplianCe, with the terms of the Operational Agreement shall subject the Pernnittee to all sanctions provided by North Carolina General Statutes § 143-215.6A to § 143-215.dC for violation of or failure to act In accordance with. the ' terms and conditions of this permit. 4. This permit is not transferable. Jn the event there Is a desire for the facilities to change ownership, or there is a name change of the Permittee, a formal permit request must be submitted to the Division accompanied by an application fee, documentation from the parties involved, and other supporting materials as may be appropriate. The approval of this guest will be considered on its merits and may. or may not be approved. 5. Failure to abide by the conditions and limitations contained in this permit may subject the Permittee- to an enforcement action by the Division in accordance with North Carolina General Statute 143-215.6A to 143-215.6C. 6.
