North Carolina Administrative Code 15A NCAC 2B .0506 (a) (2) states that "Every person subject to this
Section shad report by telephone to either the central office or appropriate regional office of the Division as
soon as possible but no later than 24 hours after occurrence or on the next working day following the
occurrence of first knowledge of the occurrence of ... any failure of a ... treatment facility resulting in a by-pass
without treatment of all or any portion of the wastewater.' Item (3) states that `Persons reporting such
occurrences by telephone shall also provide a written'report to the Division in letter form setting out
the... pertinent information pertaining to the occurrence. This report must be received by the Division within
five days following fast knowledge of the occurrence.' Non -Discharge Permit No. WQ0002284 also requires
24-hour notification and a written report within five days.
No reports of bypass have been received by the Division in 2001. If no bypass events occurred, simply
submit a written report stating so. If bypass events have occurred, you should submit to the Division
of Water Quality Washington Regional Office within ten (10) working days a written report outlining any
treatment unit bypass events that may have occurred during June, July, August and September 2001.
Include the dates and duration of any treatment unit bypass events including but not limited to tertiary
ffter bypass events. Also .provide any actions that have been taken or are proposed to Insure that
similar events do not recur. Should you have any questions, please do not hesitate to contact Kim
