North Carolina Administrative Code 15A NCAC 2B .0506 (a) (2) states that `Every person subject
to this Section shall report by telephone to either the central office or appropriate regional office
of the Division as soon as possible but no later than 24 hours after occurrence or on the -next
working day fig the occ unence of first Imowiedge of the occurrence of_..any failure of.

provide a written report to the Division in letter form setting out the... pertinent information

following first knowledge of the occurrence.' Non -Discharge Permit No. W00002829 also
requires 244xx r notification and a written report within five days.
No reports of bypass have been recehred by the Division in 2001. If no bypass events
occurred, Simply submit a vffMen report stating so. If bypass events have occurred, you
should submit to the Division of Water Quality Washington Regional Office within ten (10)

occurred during June, may, August and Septmnber 2001. Include the dates and duration of
