WASTEWATER COLLECTION SYSTEM
I, Mark S. Bissell, P.E., as a duly registered Professional Engineer in the State of
North Carolina, having been authorized to periodically observe the construction of the
project known as the Wastewater Collection System Extension - for the Lake Side
Village at Kinnakeet Shores, Dare County, North Carolina hereby state that, to the
best of my knowledge and belief, based upon the limited observation provided, a
review of the contractor's records of construction and final inspection of the project
as constructed, due care and diligence were used in the project construction such that
the project was built within substantial compliance and intent of the plans and
specifications as approved by the N.C. Division of Environmental Management under
Permit No. W00002296 Amendment dated November 30, 1993 subject to the
following:
1. As -built sewer line grades are indicated on the attached
record drawing dated March 3, 1994.
