Powers SUBJECT: Kinnakeet Shores WWTP 220,000 GPD Expansion Dare County GW# 88338 WQ Permit No. 15514 This office has reviewed the revised data, location, and modeling of the above site. We have no objection to issuance of an approval for expansion provided that: 1. Four (4) monitor wells shall be constructed on the Perimeter of Compliance boundary (250 feet from the edge of the source). As this office is not satisfied with the proposed monitor locations provided by the consultant, the location of the wells must be approved by the Washington Regional Office prior to their construction. It is our intent to locate the wells in such a manner that one upgradient and three downgradient will be provided for groundwater monitoring of the site. 2. The four monitor wells shall be sampled initially after construction, then every January, April, July, and October thereafter. Parameters to be analyzed should include, but not be limited to: Nitrates Total Ammonia Total Dissolved Solids pH Total Organic Carbon Chlorides Orthophosphates Total Organic Halides (initially and then in Octobers only) Total Coliforms Water Levels (prior to sampling) 3. A piezometer well shall be constructed in the center of the four rotary distributor cluster. The piezometer shall be monitored for water level elevations only. Water levels shall be obtained and reported every January, April, May, June, July, August, September, and October. Bill Reid Page 2 August 7, 1989 4. The measuring points on all wells shall be surveyed to the nearest 0.01 foot above mean sea level and those measuring point.elevations provided to the Department. The permit should reflect all the standard clauses for groundwater protection. All analysis and water level reporting should be on the GW-59 report forms. A question was raised in discussion with Rudy Smithwick concerning the establishment of a review boundary. If a review boundary is necessary, then the four monitor wells should be moved to that point instead of the Perimeter of Compliance. Rinnakeet Shores may also need to develop a conceptual remedial action plan for addressing the potential of failure of the disposal method. Please contact me for any further information.
