Please take note of the following comments regarding the permit application contents:
1. The budget for FY 23-24 for Utility Lines includes funding for both water and sewer lines. Half of the
total amount has been included in the application. The budget for the City's duplex lift stations is also
included on a separate sheet as part of Appendix C.
2. The CIP also covers both water and sewer lines. Half of the total amount has been included in the
application.
3. Each application includes an 8 %: X 14-inch copy of the collection system map from the ArcGIS mapping
system used by the City. When using the system, more specific information is available for each system
structure.

Division of Water Resources INSTRUCTIONS FOR FORM CSA 04-16 & SUPPORTING DOCUMENTATION
Documents shall be prepared in accordance with 15A NCAC 02T .0100, 15A NCAC 02T .0400, and all relevant
Division Policies. Failure to submit all required items will necessitate additional processing and review time.

General — When submitting an application to the Municipal Permitting Unit, please use the following instructions as a
checklist in order to ensure all required items are submitted. Adherence to these instructions and checking the provided
boxes will help produce a quicker review time and reduce the amount of requested additional information.
The Applicant shall submit one original and one copy of the application and supporting documentation.
The copy may be submitted in digital format.
A. Cover Letter
® Submit a cover letter listing all items and attachments included in the permit application package
B. No Application Fee Required
➢ No application fee is necessary. The permittee will be billed an annual fee upon issuance of the permit

C. System -Wide Wastewater Collection System (FORM: CSA 04-16) Application:
® Submit the completed and appropriately executed System -wide Wastewater Collection System (FORM: CSA 04-
16) application. Any unauthorized content changes to this form shall result in the application package being
returned. If necessary for clarity or due to space restrictions, attachments to the application may be made, as long
as the attachments are numbered to correspond to the section and item to which they refer.
❑ If the Applicant Type in Section I.3 is a Privately -Owned Public Utility, provide the Certificate of Public
Convenience and Necessity (CPCN) from the North Carolina Utilities Commission demonstrating the Applicant is
authorized to hold the utility franchise for the area to be served by the wastewater collection system, or
❑ Provide a letter from the North Carolina Utilities Commission's Water and Sewer Division Public Staff stating an
application for a franchise has been received and that the service area is contiguous to an existing franchised area
or that franchise approval is expected.
❑ If the Applicant Type in Section I.3 is a corporation or company, provide documentation if it is registered for
business with the North Carolina Secretary of State.

➢ The Authorized signing official listed in Section I.4 should match with that of the Applicant certification page in
accordance with 15A NCAC 02T .0106(b). Per 15A NCAC 02T .0106(c), an alternate person may be designated

.0106(b).

V. COLLECTION SYSTEM ADMINISTRATION:
1. Provide a brief description of the organizational structure that is responsible for management, operation and maintenance of
the collection system.
The Collection System is staffed by nine (9) full time personnel. The City's Director of Public Works and Utilities is the
ORC for the system. Immediately under the Director, is the Utility Lines Supervisor, and the remainder of staff report to this
Supervisor. The Collection System staff is responsible for the routine inspection, maintenance and cleaning, as well as repair and
uungrading of the collection lines, manholes, and connections. The lift stations are operated, maintained and repaired by four (4)
members of the Wastewater Treatment staff who report to the Wastewater Manager and ORC of the WWTP. The ORC of the WWTP
also reports to the City's Director of Public Works and Utilities.

Collection System Spill Response, Evaluation and Remedial Action Plan
I. RESPONSE PLAN
A. IDENTIFY SPILL: A spill will be identified/noticed by
1. City employees during inspections, repairs, routine visits, etc.
2. Community citizens
B. REPORT SPILL INTERNALLY AND RESPOND
1. Spills will be reported to the appropriate supervisors, ORCs, per the Contact List
2. Whenever feasible, a City employee should respond to a spill immediately, not to exceed
15 minutes.
C. SPILL SITE ASSESMENT
1. Site will be assessed within 60 minutes of knowledge of spill.
2. Will take note of source, cause, size, type, ease to access the source, etc.
D. CORRECTIVE MEASURES
1. Sewer Line
a) Blockage
(1)Dislodge Blockage.
(2)Inspect for other blockages.
(3)Ensure normal flow is resumed.
b) Breakage
(1)lsolate source of spill.
(2)Temporarily stop spill with sandbags, concrete, or other means.
(3)Use tin, wire, concrete, clamps, new line, as needed to permanently
repair breakage in line or,
(4)Use saddle to repair.
Pump Station
a) Pump, Electrical or Control Failure
(1)Call the 24-hour emergency septic hauler or use the City's Vacall truck to
reduce the volume of wastewater.
(2)Call the electrician as needed.
(3)Sandbag area to confine the spill and do not allow to enter a waterway.
(4)Install ABS spare sump pump into wet well and temporarily discharge
after the check valve assembly to keep up with the wastewater entering
the pump station wet well.
E. INITIAL SPILL REPORTING
1. If the spill was over 1000 gallons on land, or any volume and entered any waterways, call
the NCDEQ-DWR Fayetteville Regional Office at (910) 433-3300 to report all details of the
spill to any available staff member as soon as possible, but no later than 24 hours after. If
the spill occurs outside of normal business hours, contact the Division of Emergency
Management at telephone number (800) 858-0368 or (919) 733-3300. Obtain a BIMS
number during the reporting in order to complete the required written spill report.
F. LOG the incident in the appropriate logbook(s).
G. COLLECT the appropriate up and downstream samples if needed.
H. CLEANUP -Refer to "Recommended Clean-up Procedures" on Page 4.
I. ADDITIONAL SPILL REPORTING
1. If the spill was 1000 gallons or greater into waters of the state, a press release must be
made within 24 hours. See Section V for form and content required.

2. If the spill was 15,000 gallons or greater into waters of the state, public notice is required
within 10 days of the determination of affected counties. See Section VI for form and
content required.
3. The authorized individual must complete the BIMS report on the NCDEQ BIMS reporting
website within 5 days of the incident.
a) The link to the BIMS reporting site is listed on the NCDEQ, Water Resources
webpage for Collection Systems, or users may type in:
https://bimsportal.ncdenr.org/Welcome.do
b) The user ID is the City's Collection Systems permit number, and the password is
"spills".
c) Follow the portal prompts to complete the online report. Once submitted online,
print a hard copy, and keep in the City's Spill Response Plan book.
SITE REINSPECTION
1. Re -inspect the spill site within 24 hours to ensure that the repair or corrective measure
remains secure.
2. If necessary, take additional samples up and downstream to ensure that stream
standards are met.
K. PUBLIC EDUCATION
1. If the sanitary sewer overflow (SSO) is the result of grease or unpermitted items (diapers,
wipes, etc.), distribute the City's informational flyers to residences and businesses in the
area.
2. Attempt to locate the source of the unpermitted items.
3. Continue to follow-up with business and residents in the area to ensure compliance.

II. RECOMMENDED CLEAN-UP PROCEDURES
A. Remove all visible debris.
B. Remove all visibly ponded wastewater.
C. Disinfect the affected area (NO CHEMICAL ADDITION TO SURFACE
WATERS).
D. Barriers (i.e., colored tape) should be erected to preclude public
access for at least 24 hours.
E. If soil is disturbed in close proximity to stream banks, the area should
be reseeded immediately to prevent erosion.

III. LIFT STATION CONTINGENCY PLAN
A. GENERAL INFORMATION
1. The City owns and operates 15 major duplex lift stations.
2. Refer to page 16-17 for a table of lift station information.
3. The major lift stations are operated and maintained by the WWTP staff.
B. IDENTIFIERS AND ACCESS
All lift stations are clearly and conspicuously marked with the lift station name and
contact information for emergencies.
All lift stations have restricted access with keyed lock entry by Public Works Staff
C. RELIABILITY
1. All lift stations are duplex, with each pump capable of pumping at a rate of 2.5 times the
average daily flow rate with the largest pump out of service.
2. In addition, 3 portable pumps are maintained at the WWTP for back-up.
3. 7 of the 15 lift stations have back-up generators on site. Portable generators are also
maintained at the WWTP for the existing stations.
4. All lift stations are equipped with visual and audible alarms and either auto -dialer
telemetry or SCADA.
D. INSPECTIONS AND CLEANING
1. WWTP staff perform weekly inspections and cleaning of the lift stations per section II(4)
of the permit.
2. All lift stations are jet -vacuumed semi-annually to annually.
SPILLS / LIFT STATION FAILURES
1. In general, any and all spills involving failure at a lift station should be addressed as any
other SSO (see I. Response Plan and 11. Recommended Clean Up Procedures).
2. In the event of a pump failure, the second pump should be sufficient for maintaining

value. After unstopping, washing the line, and allowing the flow to equalize, estimate ibe normal flow depth in ft line and find the
