We find the plan to be acceptable with modifications and hereby issue this letter of Approval with Modifications. The Modifications Required for Approval are listed on the attached page. This plan approval shall expire three (3)years following the date of approval, if no land-disturbing activity has been undertaken,as is required by Title 15A NCAC 4B .0129. As of April 1, 2019, all new construction activities are required to complete and submit an electronic Notice of Intent(eNOI) form requesting a Certificate of Coverage (COC)under the NCG010000 Construction General Permit. After the form is reviewed and found-to be complete,you will receive a link with payment instructions for the $100 annual permit fee. After the fee is processed,you will receive the COC via email. As the Financially Responsible Party shown on the FRO form submitted for this project,you MUST obtain the COC prior to commencement of any land disturbing activity. This approval is for the work shown on the drawings received by our office on June 14, 2023. No site grading was shown. Additional erosion and sediment control plans will be required for site grading and development beyond that shown on these plans. • 2. Based on the contour lines shown on Sheet C4.0, it appears that a portion of water from Lots 1 and 2 will not get into the basin and will pool in place. A design modification in this area may be required. 3. It is recommended that the diversion berm on the west side of the site be extended approximately 140 feet to the north to route additional runoff to the basin. 4. The limits of disturbance (LOD) as shown on Sheet C4.0 does not include the sewer connections shown on Sheet C 1.0. The LOD shown on Sheet C4.0 must be modified to include the ground disturbance for the sewer lines. Additionally, Sheet C4.0 needs to be modified to note that the DEQ Inspector should be invited to the pre-construction meeting. Furthermore, Sheet C4.0 shows a "proposed silt fence" callout on the northeast side of the site,however,the silt fence is not shown. This should be corrected. Lastly,the LOD on the eastern side of the site is shown beyond the right of way. The LOD should be entirely on property owned by the financially responsible party, or, within the right of way. If the LOD is shown on property owned by others,property owner permission would be required. Correct this drawing issue. Submit two copies of the updated Sheet C4.0 to this office within 2-weeks of the date of this letter, or,before starting work,whichever is sooner. 5. Silt fence outlets are not shown on Sheet C4.0. Install silt fence outlets as needed in low areas. 6. Work is shown within the rights-of-way for No Mans Avenue and for Fisher Street. All necessary easements and access agreements must be obtained and be current prior to starting work. 7. Additional measures may be required - The applicant is responsible for the control of sediment on- site. If the approved erosion and sedimentation control measures prove insufficient,the applicant must take those additional steps necessary to stop sediment from leaving this site.
