My apologies for the delay. All this rain has put me in the field a lot lately and making me fall behind on my
office work.
Attached you will find the PCN attachments. Not sure how it got left out. As part of the attachments, you will
find the Official Species List and NLEB Consistency Letter. Please let me know if you need anything further,
with regard to T&E.
Since the previous permit was issued and as a result of increased residential growth in the area, the demand
and need for medical services has grown. At this time, the Applicant does not intend to impact anymore
wetlands for this project, thereby avoiding impacting approximately 0.40-acre of wetlands, located on the tract
to the east of the current project site.

Hi Dana,
I'm not seeing anything in the file on our end other than the PCN application form, submitted 5-23-2023 — please send us
the rest of the PCN application package. Also, I see that a previous permit was referenced for the ESA; however, species
have been uplisted since then, so you'll need to address those. Also, additional information regarding avoidance and
minimization is required, as there was a permit previously submitted for this project it was addressed at that time included
this additional area.
