Meetinq Minutes 7/8/15 24138 Site 4, 5, 6, 7, 8
Site #4: The plans and permits were reviewed on-site at this location. No pipe stakes were
installed therefore Flatiron/elythe was notified to stake this area several days before laying the
pipe to make any field adjustments needed (the alignment of the pipe coming into the existing
stream channel should be adjusted both horizontally and vertically if needed to ensure the
banks do not wash out or a scour hole doesn't form in the creek channel). There was not a
construction sequence attached in the RFC plans. Once the pipe is sraked Flatiron/Blythe will
submit a plan for NCDOT/NCDNER review. It was noted that this was a dry stream channel.

pipe to make any field adjustments needed (the alignment of the pipe coming into the existing

sequence was reviewed and will be followed prior to installing the proposed pipe.

stream. Flatiron/Blythe was still reminded that sediment needs to remain within the job limits
and out of the Jurisdictional area below the pipe outfall. Plan measures need to be installed and
maintained during construction.

was reviewed and will be followed prior to installing the proposed culvert. The culvert will be
installed followed by stream mitigation. There was a question about the temporary excavation
in the wetland area. Todd Collins is going to follow up with RKK concerning this matter to see if
this additional work is needed.
• If any field changes are made prior to the pipe being installed (alignment or elevationsJ
an email will be sent to RKK/NCDNER/NCDOT for their records.
• If the construction sequence deviates from the plans or what was discussed in the field a
new plan will be submitted similar to previous construction submittals.
