Responsible Person should be Harrison Smith and Reid Smith should print his name and title on the "Print Name &Title
of Signed if Authorized" line.This will give Reid the authority to sign the NOT Certification Form and submit the eNOT.
Invoice due date=8/1/2022, project closeout date= 7/26/2023. Please pay past due invoice prior to resubmitting eNOT
to rescind COC.
