Permit conditions from DEQ DWR's 401 water quality certification permit (if applicable to this project) require that the NCDOT Division Environmental Supervisor or Environmental Assistant will conduct a preconstruction meeting with all appropriate staff to ensure that the project supervisor and essential staff understand the potential issues at the permitted site. 303(d) IMPAIRED WATERS LIST: The Calabash River, which has been identified on the 303(d) list of impaired waters as impaired for sedimentation and/or turbidity, is within one mile of the project and receives drainage from the project. The Contractor shall adhere to all conditions and/or regulations required for impacts to these waters. This designation requires special procedures to be used for clearing and grubbing, temporary stream crossings, and grading operations within the High Quality Water Zone and as designated by the Engineer. The High Quality Water Zones are identified on the plans as Environmentally Sensitive Areas. This also requires special procedures to be used for seeding and mulching and staged seeding. The High Quality Water Zone/Environmentally Sensitive Area shall be defined as a 50-foot buffer zone on both sides of the stream measured from top of streambank. MINIMIZE REMOVAL OF VEGETATION: The Contractor shall minimize removal of vegetation within project limits to the maximum extent practicable. Vegetation along stream banks and adjacent to other jurisdictional resources outside the construction limits shall only be removed upon approval of Engineer. No additional payment will be made for this minimization work. This submittal number shall not have suffixes other than those reflecting resubmittals of the same information. Specifically, "Revise and Resubmit" submittal responses require correction and resubmittal with the original submittal number and an R suffix. For example, submittal S-001 shall be revised to S-OOIRI to reflect the first resubmittal and S-OO1R2 to reflect the second resubmittal of submittal S-001. • Submittals shall contain information for only one discipline. For example, if a structure submittal and roadway submittal are submitted on the same day, two separate submittals shall be required. The department will then either review the submittal in house or forward the submittal to the appropriate reviewing personnel. • The submittal response will include a standard response that indicates whether the department has comments or requires a resubmittal on that item. • Submittals shall be transmitted in a logical order and in accordance with the project CPM schedule most recently submitted. • Send submittals in priority order as needed on the project and looking at lead times, manufacturing, etc. Emailing submittals will expedite the review and approval process. Since NCDOT is the license holder, the contractor will not need to purchase a license. When submitting pricing, submit a detailed breakdown of costs for each contractor/subcontractor involved in the work. If pricing is submitted as a "lump sum" the request will be rejected and not reviewed until a detailed breakdown is provided, with the exception of line items with a lump sum unit of measurement. To assist with the review and turnaround time of the submitted prices, the submittals should be in the form of force account records, containing personnel, materials, equipment, markups, etc. Claims Work to eliminate Claims All issues/concerns should be handled in the field If no resolution can be made, then elevate Roadway PRICE ADJUSTMENT - ASPHALT BINDER FOR PLANT MIX: Price adjustments for asphalt binder for plant mix will be made in accordance with Section 620 of the 2018 Standard Specifications. The base price index for asphalt binder for plant mix is $ 641.88 per ton. This base price index represents an average of F.O.B. selling prices of asphalt binder at supplier's terminals on April 1, 2023. HIGH STRENGTH CONCRETE FOR DRIVEWAYS: Use high early strength concrete for all driveways shown in the plans and as directed by the Engineer. Provide high early strength concrete that meets the requirements of Article 1000-5 of the 2018 Standard Specifications. Measurement and payment will be in accordance with Section 848 of the 2018 Standard Specifications. MATERIAL AND EQUIPMENT STORAGE & PARKING OF PERSONAL VEHICLES: When work is not in progress, keep all personnel, equipment, machinery, tools, construction debris, materials and supplies away from active travel lanes that meets Table 1101-1. PATH LIGHTING SYSTEM 1. DESCRIPTION The work covered by this section consists of furnishing and installing LED step light luminaires, control panel, conduit system, and wirings embedded in the vertical concrete barrier rail. The Contractor actually performing the work described in these special provisions shall have a license of the proper classification from the North Carolina State Board of Examiners of Electrical Contractors. The licensed Electrical Contractor must be available on the job site when the work is being performed or when requested by the Engineer. The licensed Electrical Contractor shall have a set of plans and special provisions in his possession on the job site and must maintain accurate "as built" plans. The contractor shall provide as -built plans per Subarticle 1400-3(D) of the Standard Specifications prior to project inspection by the Lighting Team. The contractor team actually performing the electrical work shown in the plans and described in these provisions shall be on site during inspection of the lighting system. WRE to follow up Structures CARBON FIBER REINFORCED POLYMER (CFRP) PRESTRESSED CONCRETE PILES Provide 20" concrete piles that are prestressed with carbon fiber reinforced polymer (CFRP) strands in accordance with the contract plans, Standard Specifications, and Special Provisions. Use only non-metallic inserts and lifting devices with CFRP prestressed members. UBO utilities shall coordinate construction activities with contractor and NCDOT resident engineers office from date of availability to August 18, 2023 The conflicting facilities of these concerns will be adjusted prior to the date of availability, unless otherwise noted and are therefore listed in the special provisions for the benefit of the Contractor. All utility work listed herein will be done by the utility owners. All utilities are shown on the plans from the best available information. Utilities MATERIALS PROVIDED BY NCDOT NCDOT has acquired, is storing, and will assist in coordinating delivery of the materials listed in Table 1 below to the contractor for use in performance of the work. The contractor shall be responsible for transporting materials to the project site, including any coordination with NCDOT, loading, unloading, labor, and equipment necessary. The materials listed in Table 1 are stored with NCDOT's vendor. The contractor shall be responsible for coordinating delivery with NCDOT's Resident Engineer or assigned representative to ensure timely delivery as to not impact project schedule. Upon delivery it is the contractor's responsibly to take possession of the materials by inspecting, unloading, and properly storing the materials until installed.
