Division of Water Resources' Central and Regional staff has reviewed the application package
received January 26, 2023. However, additional information is required before the review may be
completed. Please address the items on the attached pages no later than the close of business on April 21,
2023.

rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
in Sections A through B, or failure to provide the additional information on or before the above requested
date may result in your application being returned as incomplete pursuant to 15A NCAC 02T .0107(e)(2).

A. Monitoring Wells:
1. It is the Division's understanding that monitoring wells MW-1 and MW-2 have been constructed.
However, no well construction records have been submitted. Please submit a Well Construction
Form (GW-1) for each well.
B. Site Map:
1. The site map that was submitted as apart of the renewal application for Permit WQ0036210 is not
up to date. Please submit two original copies and one electronic copy of a site map with a scale no
greater than 1-inch equals 100 feet. As stated in Condition I.3 of your permit, the map shall include
the following information:
i. The location and identity of each monitoring well.
ii. The location of major components of the waste disposal areas.
iii. The location of property boundaries within 500 feet of the disposal areas.
iv. The latitude and longitude of the established horizontal control monument.
v. The elevation of the top of the well casing (i.e. measuring point) relative to a common
datum.
vi. The depth of water below the measuring point at the time the measuring point is
established.
vii. The location of compliance and review boundaries.
viii. The date the map is prepared and/or revised.
The physical copies of the site map should be mailed to the Division of Water Resources, Water
Quality Permitting Section, 1617 Mail Service Center, Raleigh, NC 27699-1617. The electronic
copy should be submitted to the attention of Leah Parente at:
