Dear Mr. Locklear:
On July 28, 2023, Division of Water Resources (DWR) Winston-Salem Regional Office (WSRO)
staff Jesse Barnes conducted a routine compliance evaluation inspection at the single-family
residence wastewater treatment system located at 187 Marajo Ct, Winston-Salem, NC 27127 in
reference to NPDES general permit NCG550000 and certificate of coverage NCG550094. Mr.
Zachery Locklear, owner, was present during the inspection. A review of the subject permit,
structures, treatment components, operations, and maintenance was completed. This review
reflected noncompliance with the subject permit. Items of concern observed during the inspection
are listed below.
Items of Concern
Mr. Locklear stated that no effluent monitoring has been conducted, as is required by Part
Section C. (1.) of the subiect
2. No tablets were present in the chlorinator tubes during the inspection. Part I, Section B. (L)
of the subject permit requires that the treatment system includes disinfection. Part I, Section
D. (L) of the subject permit requires that the tablet chlorinator be inspected weekly to ensure
there is an adequate supply of tablets for continuous and proper operation.

Mr. Locklear stated that he did not know when the septic tank was last pumped or inspected.
Part I, Section D. (3.) of the subject permit requires septic tanks to be inspected at least
yearly to determine if solids must be removed or if other maintenance is necessary.
Additionally, Part I, Section D. (3.) requires septic tanks to be pumped out at least once
every five years or when the solids level is found to be more than 1/3 of the liquid depth in
any compartment, whichever is greater.
Please be aware that the Division may take enforcement action against any permittee who fails to
comply with any requirement of their permit. Pursuant to NC General Statute (NCGS) 143-215.6A,
a civil penalty of not more than twenty-five thousand dollars ($25,000.00) may be assessed against

any person who violates or fails to act in accordance with the terms, conditions, or requirements
of any permit issued pursuant to G.S. 143-215.1.

Other observations:
Two tablet chlorinator tubes were observed during the inspection and appeared to be operating
properly. Mr. Locklear stated that he typically applies two or three tablets monthly. The tablets Mr.
Locklear typically uses were inspected and observed to contain the active ingredient calcium
hypochlorite, which is appropriate for domestic wastewater treatment.
The effluent outfall and receiving stream were observed during the inspection and located
immediately adjacent to the subject parcel. The white plastic outfall pipe was visible and appeared to
be operating properly. No effluent flow was observed during the inspection, but Mr. Locklear stated
that he has observed occasional effluent flow. No odors, solids, foam, nor debris were observed at
the effluent outfall to the receiving stream during the inspection.
