Main Street Yanceyville, NC 27379 Dear Mr. Davis: PERMITS & ENGIN ERIi SUBJECT: Permit No. NCO040011 Authorization to Construct Yanceyville Sanitary District Yanceyville Wastewater Treatment Facility Improvements EPA Project No. C370494-02 The final plans and specifications for the subject project have been reviewed and found to be satisfactory. Authorization is hereby granted for the upgrade of the existing 0.125 MGD single cell stabilization lagoon to a 0.20 MGD oxidation pond to consist of a new manual bar screen, three (3) five (5) HP surface aerators, new lagoon influent structure, effluent polishing sand filter (gravity high rate type) with clearwell, two (2) 660 gpm backwash pumps, an air scour backwash system, backwash wetwell with two (2) 150 gpm backwash return submersible Pumps, intermittent sand filter for recycled backwash, prefabricated chlorinator building with gas chlorination system, approximately 1130 l.f. of 10-inch effluent lie, cascade reaeration, approximately 1020 l.f. of 4-inch force main, effluent flow metering device, access road, appurtenances, and electrical work. Do not proceed with construction until the Authorization -to -Award package, EEO, and MBE documentation/certification have been reviewed, and you are in receipt of our approval if Federal Funding is desired for project construction. Box 27687, Raleigh, North Carolina 27611-7687 Telephone 919-733-7015 An Equal Opportunity Affirmative Action Employer The Permittee shall employ an actively State certified wastewater treatment plant operator to be in responsible charge of the wastewater treatment facilities. Such operator must hold a valid State certificate of the grade at least equivalent to the classification assigned to the wastewater treatment facilities by the Certification Commission. When a new chief operator must be obtained to be in responsible charge of the new facilities, it is required that this be done by the time the construction of the facili- ties is 50% completed. This Authorization -to -Construct is issued in accordance with Part III, Paragraph C of NPDES Permit No. NC0O4O011 issued June 29, 1984, and shall be subject to revocation unless the wastewater treatment facilities are constructed in accordance with the approved plans, specifications, and supporting documents, and are in compliance with the conditions and limitations specified in Permit No. NC0040011. During the construction of the proposed additions/modifications, the Permittee shall continue to properly maintain and operate the existing wastewater treatment facilities at all times, and in such manner, as necessary to comply with the effluent limits specified in the NPDES Permit. You are reminded that it is mandatory for the project to be constructed in accordance with the North Carolina Sedimentation Pollution Control Act; and when applicable, the North Carolina Dam Safety Act. In addition, the specifications must clearly state what the contractors' responsibilities shall be in complying with these acts. The selling or free distribution of wet, dried, composted sludge or sludge -like materials from wastewater facilities without prior review and approval by this Division is illegal. One (1) set of final approved plans and specifications is to be forwarded to you, one (1) set to the Environmental Protection Agency (EPA) , and two ( 2 ) sets to the Corps of Engineers.
