Dear Mr. Davis:
The final plans and specifications for the subject project have been
reviewed and found to be satisfactory.
Authorization is hereby granted for the upgrade of the existing
0.125 MGD single cell stabilization lagoon to a 0.20 MGD oxidation pond
to consist of a new manual bar screen, three (3) five (5) HP surface
aerators, new lagoon influent structure, effluent polishing sand filter
(gravity high rate type) with clear well, two (2) 660 gpm backwash pumps,
an air scour backwash system, backwash wetwell with two (2) 150 gpm back-
wash return submersible pumps, intermittent sand filter for recycled
backwash, prefabricated chlorinator building with gas chlorination
system, approximately 1130 1.f of 10-inch effluent line, cascade re -
aeration, approximately 1020 l.f of 4-inch force main, effluent flow
metering device, access road, appurtenances, and electrical work.
Do not proceed with construction until the Authorization -to -Award
package, EEO, and MBE documentation/certification has been reviewed, and
you are in receipt of our approval if Federal Funding is desired for
project construction.

The Permittee shall employ an actively State certified wastewater
treatment plant operator to be in responsible charge of the wastewater
treatment facilities. Such operator must hold a valid State certificate of
the grade at least equivalent to the classification assigned to the wastewater
treatment facilities by the Certification Commission. When a new chief
operator must be obtained to be in responsible charge of the new facilities,
it is required that this be done by the time the construction of the facili-
ties is 50% completed.
This Authorization -to -Construct is issued in accordance with Part III
Paragraph C of NPDES Permit No. NCO040011 issued June 29, 1984, and shall
be subject to revocation unless the wastewater treatment facilities
are constructed in accordance with the approved plans, specifications,
supporting documents, and are in compliance with the conditions and
limitations specified in Permit No. NCO04,0011.
During the construction of the proposed additions/modifications,
the Permittee shall continue to properly maintain and operate the
existing wastewater treatment facilities at all times, and in such
manner, as necessary to comply with the effluent limits specified in the
NPDES Periait.
You are reminded that it is mandatory for project to be constructed in
accordance with the North Carolina Sedimentation Pollution Control Act; and
when applicable, the North Carolina Dam Safety Act. In addition, the
specifications must clearly state what the contractors responsibilities
shall be in complying with these acts.
The selling or free distribution of wet,._.dried, composted sludge or
sludge like materials from wastewater facilities without prior review and
approval by this Division is illegal.
One (1) set of final approved plans.and specifications is to be
forwarded to you, one (1) set to the Environmental Protection Agency (EPA),
and two (2) sets to the Corps of Engineers.

An erosion and sedimentation control plan has not been submitted to
the Division of Land Resources, WSRO, as of this date.
If any of the existing treatment units, e.g. chlorination, will be
taken out of service during construction, a plan of.action to minimize
environmental..impacts should be developed.
The potential bypassing of the tertiary filter should be clarified.
In the event of a power failure, the filter should bypass to the chlorine
contact chamber. If the filter backwash pump wet well f ills• due to pump
failure(s) while the filter itself continues to function, what couse of
action will be taken? The possibility of wet' well overflows should be taken
into account in designing the filter -system and planning its operation.
The improvements at the subject facility are greatly needed. The
Winston-Salem Regional Office recommends that the Yanceyville Sanitary
District receive a grant for the proposed improvements as soon as grant
requirements are met.

The following information is submitted per your August 20, 1985 checklist
comments:
1. The Winston-Salem Regional Office has sent you information on
the NPDES Permit Status. -- A& -r TeuL= Strvr 70
2. The Corps of Engineers has advised us that at Sector 404/
Section 10 Wetlands Permit is not required for this project
(copy of Sept. 19, 1985 letter attached).
3. No N.C. D.O.T. or railroad encroachments are required as a
part of this project.
4. The District owns in fee simple all land where the treatment
plant improvements are to be constructed.
5. The area to be disturbed as a part of this project is in areas
containing less than one contiguous acre; therefore formal
plan approval is not required. However, we have incorporated
erosion control measures in the plans.
6® Flood insurance is not required for this project.

On September 13, 1985, you spoke with Mr. Jeff Richter of my
staff concerning the Town of Yanceyville's proposed plans to
upgrade its wastewater treatment plant on Jail House Branch and
Country Line Creek south of Yanceyville, Caswell County, North
Carolina.
During this conversation you discussed with Mr. Richter the
Corps of Enginers' letter of September 11, 1985, to you about how
a Department of the Army permit is required for fill material
placed in wetlands. You said that no fill would be placed in any
creek or water, that the outfall pipe in the plans would come down
a steep hill from the existing facility and stop at the creek.
This pipe would not be covered over, and the slope would be seeded
immediately following construction in order to prevent erosion
back into the creek.
Mr. Richter visited the site on April 17, 1985, and had
.determined that no wetlands exist in the vicinity of the proposed
project, therefore no Department of the Army permit will be
required.. This, however, does not relieve you of the
responsibility to obtain any other required Federal, State, or
local authorization.

MEMORANDUM

SUBJECT: Wastewater.Treatment Plant Improvements
Yanceyville Sanitary District
Caswell County
Engineers of this Section have reviewed the plans and specifications for
the subject project prepared by Alley, Williams, Carmen, and King, Incorporated.
The plans call for improving the existing Yanceyville Sanitary District's
wastewater treatment plant by installing new manually cleaned bar screens, three
(3) 5-horsepower motor aerators at the oxidation pond, a factory built steel
tertiary filter with integral clearwell and two (2) 660 gpm each backwash pumps,
two (2) new Sanuril Model No. 1000 tablet chlorinators at the chlorination
basin, a wetwell with two (2)-100 gpm each submersible pumps, sewers, manholes,
force mains, and other appurtenances. The treated effluent will be discharged
to The Country Line Creek, a Class A -II stream.
We have no objection to your issuance of'a construction permit for this
project, and shall retain the copy of the plans and specifications in our per
manent files.
