Does the submitted system comply with 15A NCAC 02T, the Minimum Design Criteria for the Permitting of Pump Stations and Force Mains (latest version), and the Gravity Sewer Minimum Design Criterig (latest version) as applicable? ® Yes ❑ No If no, for projects requiring a single variance, complete and submit the Variance/Alternative Design Request application (VADC 10-14) and supporting documents for review to the Central Office. I further attest that to the best of my knowledge the proposed design has been prepared in accordance with the applicable regulations, Minimum Design Criteria for Gravity Sewers (latest version), and the Minimum Design Criteria for the Fast -Track Permitting of Pump Stations and Force Mains (latest version). Although other professionals may have developed certain portions of this submittal package, inclusion of these materials under my signature and seal signifies that I have reviewed this material and have judged it to be consistent with the proposed design. NOTE — In accordance with General Statutes 143-215.6A and 143-215.6B, any person who knowingly makes any false statement, representation, or certification in any application package shall be guilty of a Class 2 misdemeanor, which may include a fine not to exceed $10,000, as well as civil penalties up to $25,000 per violation. The YVSA maintains the sewer collection system in the towns of Elkin, Jonesville, and Ronda (Wilkes County). The purpose of the modification to approved Permit No. WQ0043669 is to add an additional 364' of new 8" gravity sewer to replace a common 4" service line that several buildings (businesses) share. The revised and final quantities to be permitted are as follows: Previously Permitted Modification All Items to be Permitted 415 LF of 8" PVC +244 LF of 8" PVC 659 LF of 8" PVC 96 LF of 8" DIP +140 LF of 8" DIP 236 LF of 8" DIP 288 LF of 18" DIP 288 LF of 18" DIP 248 LF of 6" force main 248 LF of 6" force main All proposed gravity sewer pipe will be SDR 35 PVC or ductile iron pipe. The following from Permit No. WQ0043669 has not changed: The project also includes replacing 41 outfall manhole frame/covers with watertight frame/covers, 16 manhole height adjustments, install 10 manhole vents, 10 spot repairs of varying lengths, rehabilitation/repair of 23 manholes (including 163 VF of cementitious lining and various other items).
