This application is submitted to allow relocation of a portion of an existing permitted 16" Force
Main along/adjacent to US 401 in Fuquay-Varina.
We are only relocating a portion because of DIP material shortages that prevent us from
relocating the entire line within the time available.
We believe we can assemble enough material to relocate the Force Main out of the
intersection, where a Federally Funded Highway Improvement project is anticipated to start in
the near future. Our goal is to relocate the line before it is awarded to a contractor.
Additionally, we wish to avoid having to make pavement cuts after the intersection is
completed.
There is 0 Flow associated with this application as it is only a modification to the existing
permit.

This application is for sewer extensions involving gravity sewers, pump stations and force mains, or any combination that has been
certified by a professional engineer and the applicant that the project meets the requirements of 15A NCAC 02T and the Division's
Minimum Design Criteria (Gravity Sewer & Pump Stations/Force Mains) and that plans, specifications and supporting documents
have been prepared in accordance with 15A NCAC 02T, 15A NCAC 02T .0300, Division policies, and good engineering practices.
While no upfront engineering design documents are required for submittal, in accordance with 15A NCAC 02T .0305(b), design
documents must be prepared prior to submittal of a fast track permit application to the Division. This would include plans, design
calculations, and project specifications referenced in 15A NCAC 02T .0305 and the applicable minimum design criteria. These
documents shall be immediately available upon request by the Division.
Projects that are deemed permitted (do not require a permit from the Division) are explained in 15A NCAC 02T.0303.
Projects not eligible for review via the fast track process (must be submitted for full technical review):
➢ Projects that do not meet any part of the minimum design criteria (MDC) documents;
➢ Projects that involve more than one variance from the requirements of I5A NCAC 02T;
➢ Pressure sewer systems utilizing simplex septic tank -effluent pumps (STEPS) or simplex grinder pumps;
➢ Simplex STEP or simplex grinder pumps connecting to pressurized systems (e.g, force mains);
➢ Vacuum sewer systems.
General When submitting an application, please use the following instructions as a checklist in order to ensure all required items are
submitted. Adherence to these instructions and checking the provided boxes will help produce a quicker review time and reduce the
amount of requested additional information. Failure to submit all required items will necessitate additional processing and review
time, and may result in return of the application. Unless otherwise noted, the Applicant shall submit one original and one copy of
the application and supporting documentation.
A. One Original and One Copy (second copy may be digital) of Application and Supporting Documents
® Required unless otherwise noted. Signatures on original must be "wet ink" or secure digital signatures.
Please do not submit engineering design plans with the application unless specifically requested.
B. Cover Letter/Narrative Description (Required for All Application Packages):
® List all items included in the application package, as well as a brief description of the requested permitting action.
➢ Be specific as to the system type, number of homes served, flow allocation required, etc.
➢ Include the permit number/status of any other required sewer permits (downstream/upstream)
➢ If necessary for clarity, include attachments to the application form.
C. Application Fee (All New and Modification Application Packages):
® Submit a check or money order in the amount of $480.00, dated no more than 90 days prior to application submittal.
➢ Payable to North Carolina Department of Environmental Quality (NCDEQ)
D. Fast Track Application (Required for All Application Packages, Form FTA 05-21):
® Submit the completed and appropriately executed application.
➢ If necessary for clarity or due to space restrictions, attachments to the application may be made.
❑ If the Applicant Type in Item 1.2 is a corporation or company, provide documentation it is registered for business with the
North Carolina Secretary of State.
❑ If the Applicant Type in Item 1.2 is a partnership or d/bla, enclose a copy of the certificate filed with the Register of Deeds in
the county of business.
® The Project Name in Item 11.1 shall be consistent with the project name on the flow acceptance letters, agreements, etc.
® The Professional Engineer's Certification on Page 5 of the application shall be signed, scaled and dated by a North Carolina
licensed Professional Engineer.
® The Applicant's Certification on Page 5 of the application shall be signed in accordance with 15A NCAC 02T .0106(b). Per
15A NCAC 02T .0106(c), an alternate person may be designated as the signing official if a delegation letter is provided from
a person who meets the criteria in 15A NCAC 02T .0106(b).

E. Flow Tracking/Acceptance Form (Form: FTSE 04-16) (If Applicable):
® Submit the completed and executed FTSE form from the owners of the downstream sewers and treatment facility.
➢ Multiple forms maybe required where the downstream sewer owner and wastewater treatment facility are different_
➢ The flow acceptance indicated in form FTSE must not expire prior to permit issuance and must be dated less than one year
prior to the application date.
➢ Submittal of this application and form FTSE indicates that owner has adequate capacity and will not violate G.S. 143 215.67(a).
➢ Intergovernmental agreements or other contracts will not be accepted in lieu of a project -specific FTSE.
F. Site Maps (All Application Packages):
18 Submit an 8.5-inch x t 1-inch color copy of a USGS Topographic Map of sufficient scale to identify the entire project area,
including the closest surface waters.
➢ General location of the project components (gravity sewer, pump stations, & force main)
➢ Downstream connection points and permit number (if known) for the receiving sewer
® Include an aerial location map showing general project area (such as street names or latitude/longitude) so that Division staff
can easily locate it in the field.
G. Existing Permit (Application Packages for Modifications to an Existing Permit):
® Submit a copy of the most recently issued existing permit. not available.
® Include a descriptive and clear narrative identifying the previously permitted items to remain in the permit, items to be
added, and/or items to be modified (the application form itself should include only include items to be added/modified). The
narrative should also include whether any previously permitted items have been certified.
® The narrative should clearly identify the requested permitting action and accurately describe the sewers to be listed in
the final permit.
H. Power Reliability Plan (Required if portable reliability option utilized for Pump Station):
❑ Per 15A NCAC 02T .0305(h)(1), submit documentation of power reliability for pumping stations.
➢ This alternative is only available for average daily flows less than 15,000 gallons per day
➢ It shall be demonstrated to the Division that the portable source is owned or contracted by the applicant and is compatible with
the station. The Division will accept a letter signed by the applicant (see 15A NCAC 02T .0106(b)) or proposed contractor,
stating that "the portable power generation unit or portable, independently -powered pumping units, associated appurtenances
and personnel are available for distribution and operation of this pump station."
D If the portable power source or pump is dedicated to multiple pump stations, an evaluation of all the pump stations'
storage capacities and the rotation schedule of the portable power source or pump, including travel timeframes, shall
be provided in the case of a multiple station power outage. (Required at time of certification)
I. Certificate of Public Convenience and Necessity (All Application Packages for Privately -Owned Public Utilities):

Commission demonstrating the Applicant is authorized to hold the utility franchise for the area to be served by the sewer

I just went over the fast track application for this project. I'd like to turn this around for you folks as
quickly as we can. Do you guys happen to have a copy of the old permit? I'm having trouble locating
it here (although our computer system indicates 20,000 LF 16 in force main, 78 LF 12 in force main,
1721 LF 18 in GS, and 1260 gpm pump station- if this sounds familiar, as it is from 3/2000). Susan,
Michael is the new Director and has searched their files with no luck. I do have a copy of the

I need to have some estimation on the increased linear footage with the relocation and a good pipe
diagram of the relocation? That can be simplistic — I just need a better idea of what's entailed. I

checked the language in 15A NCAC 2T .0303(4) and (C)? Could this be deemed permitted? Scroll to
page 18. 1 lose all formatting, or I'd copy it here. Yes, both Michael and I checked and Michael
noted that we would not be entirely within the existing easement, hence we felt like we did not
qualify for the exception.

Fuquay-Varina for the construction and operation of the subject wastewater collection extension. This permit
shall be effective from the date of issuance until rescinded and shall be subject to the conditions and
limitations as specified therein.
Permitting of this project does not constitute an acceptance of any part of the plans and specifications
that do not meet 15A NCAC 2H .0200 and the Division of Water Quality's Gravity Sewer Minimum Design
Criteria adopted February 12, 1996 unless specifically mentioned herein. Division approval is based on the
acceptance of the certification provided by a N.C. Professional Engineer on Page 6 of the subject permit
application form. It shall be the permittee's responsibility to ensure the plans and specifications meet the
appropriate design criteria and rules. Failure to comply may result in penalties in accordance with North
Carolina General Statute 143-215.6A — 143-215.6C, construction of additional facilities, and/or referral of the
N.C. Professional Engineer to the licensing board.
If any parts, requirements, or limitations contained in this permit are unacceptable, you have the right
to request an adjudicatory hearing upon written request within thirty (30) days following receipt of this permit.
This request must be in the form of a written petition, conforming to Chapter 150B of the North Carolina
General Statutes, and filed with the Office of Administrative Hearings, 6714 Mail Service Center, Raleigh, NC
27699-6714. Unless such demands are trade this permit shall be final and binding.

and other applicable Laws, Rules, and Regulations

construction and operation of approximately 1,721 linear feet of 18-inch gravity sewer, a 1260-GPM pump station
with duplex pumps, on -site audible and visual high water alarms, an autodialer, an on -sight generator, an
automatic transfer switch, approximately 20,000 linear feet of 16-inch force main, approximately 78 linear feet of
• 12-inch force main, and 1700 linear feet of six-inch force main to serve The Town of Fuquay-Varina's East
Regional Pump Station, and the discharge of zero GPD (no new tributary flow) of collected municipal wastewater
into the Town of Fuquay-Varina's existing sewerage system, pursuant to the application received January 10,
2000, and in conformity with the project plan, specifications, and other supporting data subsequently filed and
approved by the Department of Environment and Natural Resources and considered a part of this permit.
This permit shall be effective from the date of issuance until rescinded and shall be subject to the following
specified conditions and limitations:
I. This permit shall become voidable unless the facilities are constructed in accordance with the conditions of
this permit, 15A NCAC 2H .0200, the Division of Water Quality's Gravity Sewer Minimum Design
Criteria adopted February 12, 1996, the approved plans and specifications, and other supporting data unless
specifically mentioned herein.
2. This permit is effective only with respect to the nature and volume of wastes described in the application
and other supporting data.
3. The facilities must be properly maintained and operated at all times.
4. The sewage and wastewater collected by this system shall be treated in the Town of Fuquay-Varina's
Terrible Creek Wastewater Treatment Plant (NPDES No. NC0066516) prior to being discharged into the
receiving stream.
5. This permit is not transferable. In the event there is a desire for the facilities to change ownership, or there
• is a name change of the Permittee, a formal permit request must be submitted to the Division of Water
Quality (Division) accompanied by an application fee, documentation from the parties involved, and other
supporting materials as may be appropriate. The approval of this request will be considered on its merits
and may or may not be approved.
6.
Construction of the sewers, pump station(s) and force main shall be scheduled so as not to interrupt service
by the existing utilities nor result in an overflow or bypass discharge of wastewater to the surface waters of
the State.
•
7.
Upon completion of construction and prior to operation of this permitted facility, a certification and a
copy of the record drawings (i.e., as -constructed plans) must be received from a professional engineer
certifying that the permitted facility has been installed in accordance with the Division's Gravity Sewer
Minimum Design Criteria adopted February 12, 1996, 15A NCAC 2H .0200, this permit, and other
supporting materials. If this project is to be completed in phases and partially certified, you shall retain the
responsibility to track further construction approved under the same permit and provide a final certificate of
completion once the entire project has been completed. A copy of the record drawings, indicating the
facilities constructed in the phase being certified, shall be submitted with each partial certification. Mail the
Certification and one (1) copy of the record drawings to the Non -Discharge Permitting Unit, 1617 Mail
Service Center, Raleigh, NC 27699-1617.
8.
A copy of the approved plans and specifications shall be maintained on file by the Permittee for a minimum
of five years from the date of the completion of construction.
9.
Failure to abide by the conditions and limitations contained in this permit or to construct the facilities in
accordance with the conditions of this permit, 15A NCAC 2H .0200, the Division's Gravity Sewer
Minimum Design Criteria adopted February 12, 1996,the approved plans and specifications, and other
supporting data may subject the Permittee to an enforcement action by the Division, in accordance with
North Carolina General Statute 143-215.6A to 143-215.6C.
10.
The issuance of this permit does not preclude the Permittee from complying with any and all statutes, rules,
regulations, or ordinances which may be imposed by other government agencies (local, state, and federal)
which have jurisdiction.
•
11.
The Permittee shall provide for the pump station and force main the following items:
a. Pump on/off elevations located so that 2-8 pumping cycles may be achieved per hour in the pump
station,
b. An air relief valve located at all high points along the force main,
c. A screened vent for the wet well,
d. Fillets located in the wet well at the intersection of the flooring and sidewalls,
e. Three feet of cover (minimum) over the force main or the use of ferrous material where three feet
cannot be maintained,
f. Sufficient devices which will protect the pump station from vandals, and
g. Flood protection if the pump station is located below the 100-year flood elevation.
h. Adherence with the following minimum separations:
(i) Storm sewers (vertical) 12 inches
(ii) Water mains (vertical -water over sewer) 18 inches or (horizontal) 10 feet
(iii) In benched trenches (vertical) 18 inches
(iv) Any private or public water supply source, including any WS-I waters or Class I or Class II
impounded reservoirs used as a source of drinking water 100 feet
(v) Waters classified WS (other than WS-I), B, SA, ORW, HQW, or SB [from normal high water (or
tide elevation)) 50 feet
(vi) Any other stream, lake or impoundment 10 feet
(vii) Any building foundation 5 feet
(viii) Any basement 10 feet
(ix) Top slope of embankment or cuts of 2 feet or more vertical height 10 feet
(x) Drainage systems •
(I) Interceptor drains 5 feet
(II) Ground water lowering and surface drainage ditches 10 feet
(xi) Any swimming pool 10 feet
(xii) Ferrous sewer pipe with joints equivalent to water main standards, shall be used where these
minimum separations cannot be maintained. The minimum separation shall however not be less
• than 25 feet from a private well or 50 feet from a public water supply well;
12. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions, the
Permittee shall take immediate corrective action, including those as may be required by this Division, such
as the construction of additional or replacement wastewater collection facilities.
13. NONCOMPLIANCE NOTIFICATION:
The Permittee shall report by telephone to the Raleigh Regional Office, telephone number (919) 571-4700,
as soon as possible, but in no case more than 24 hours or on the next working day following the occurrence
or first knowledge of the occurrence of either of the following:
a. Any process unit failure, due to known or unknown reasons, that renders the facility incapable of
adequate wastewater transport, such as mechanical or electrical failures of pumps, line blockage or
breakage, etc.; or
b. Any failure of a pumping station or sewer line resulting in a by-pass directly to receiving waters
without treatment of all or any portion of the influent to such station or facility.
Persons reporting such occurrences by telephone shall also file a written report in letter form within five (5)
days following first knowledge of the occurrence. This report must outline the actions taken or proposed to
be taken to ensure that the problem does not recur.
