We fmd the plan to be acceptable with modifications and hereby issue this letter of Approval with Modifications. The Modifications Required for Approval are listed on the attached page. This plan approval shall expire three(3)years following the date of approval, if no land-disturbing activity has been undertaken, as is required by Title 15A NCAC 4B .0129. As of April 1, 2019, all new construction activities are required to complete and submit an electronic Notice of Intent(eNOI) form requesting a Certificate of Coverage (COC) under the NCG010000 Construction General Permit. After the form is reviewed and found to be complete, you will receive a link with payment instructions for the $100 annual permit fee. After the fee is processed,you will receive the COC via email. As the Financially Responsible Party shown on the FRO form submitted for this project, you MUST obtain the COC prior to commencement of any land disturbing activity. The eNOI form may be accessed at deq.nc.gov/NCG01. Please direct questions about the eNOI form to the Stormwater Program staff the Raleigh central office. If the owner/operator of this project changes in the future,the new responsible party must obtain a new COC. Adams 1. Sheet C001 —Revise the Site Development and Erosion Control Sequence to indicate the proper order of construction(i.e., construction entrance, silt fence, sediment basins, diversions, etc.). Provide additional details that clearly describe how erosion and sediment control (E&SC) will be maintained throughout all phases of grading. 2. Sheet C400—Revise sheet(and any relevant sheets)to show the existing stormwater/drainage system for the subject properties including the culvert below Doan Drive at Pitts Road. Also, include callouts or a revised legend to indicate the existing catch basins within the limits of disturbance (LOD).are protected with silt sacks. 3. Sheet C400—Provide proper stabilization(e.g., riprap, sod, etc.)and outlet protection for the temporary diversion ditch(clean water bypass) along Pitts Road(southern tract area). 4. Sheet C400 and Sheet C401 —Revise sediment basins SSB-1 and SSB-2 so that the skimmers do not cross the baffles. Attempt to provide at least two baffles on either side of the skimmer in both basins. 5. Sheet C400—Provide temporary diversions between both sediment basins SSB-1 and SSB-2. Ensure tract runoff does not bypass the basins, and diversions have positive drainage. If these diversions need to be removed for the placement of fill as shown on Sheet C401, temporary diversions must be reinstalled at the end of every workday. 6. Sheet C001, Sheet C400 and Sheet C401 —Revise construction sequence and sheets as necessary to indicate the installation of the stormwater drainage network along Doan Drive and DA-3, DA-4, and DA-5 will be installed at the end of Phase I of the ESC and at the beginning of Phase II (when grading is conducted to allow stormwater drainage installation). 7. Sheet C401 —During the fill placement for the building pad area, if sheet flow from the graded area drains to the silt fence along Pitts Road(southern tract area), a diversion ditch/berm must be installed upstream of the silt fence to sediment basin SSB-2 to divert overland flow from the disturbed area to the basin. 8. Ensure that any required Phase II post-construction stormwater permitting, 401 certification, and 404 permitting is completed before the commencement of any grading or construction activity. Address the comments above and submit two sets of revised plans to this office on or before May 30, 2023. 9. Additional measures may be required- The applicant is responsible for the control of sediment on-site. If the approved erosion and sedimentation control measures prove insufficient, the applicant must take those additional steps necessary to stop sediment from leaving this site.
