Everything looks good except for a few small issues: Please correct the following: - Prior Comment 2 – Please upload the originally requested file at the following link: https://edocs.deq.nc.gov/Forms/SW-Supplemental-Upload. Items that were provided with the original submission but have not been provided electronically include the Application Form*, Supplement-EZ Form*, O&M Agreement Form*, USGS topo map, property deed, deed restriction documents, Secretary of State info. for the applicant, stormwater narrative, calculation booklet* plans*, etc… *The revised pages of these documents were electronically uploaded, however the unmodified pages have not been uploaded (for example, page 1 of the Application has been uploaded, since it was revised as part of the last submission, but pages 2-6 of the Application have not been uploaded). Please ensure that all items that were originally provided as part of the hard copy submission for this project are electronically uploaded. - Supplement-EZ Form, Wet Pond Page, Lines 43 & 44 – These values correspond to the distance from the permanent pool surface elevation to the elevation of the forebay entrance/exit. For example, for Wet Pond #2 (Column 1 of the form), the permanent pool elevation is at 243.5’ and the forebay entrance is at elevation 238.0’ (per the plans). This would result in a depth of forebay at entrance of 5.5’ = 66” (54” shown). Please revise as needed. Please provide the following: - 1x electronic copy of the requested files. - 1x hard copy & 1x electronic copy of the revised page of the Supplement-EZ Form. Hard copies should be mailed to me at the following address: For FedEx/UPS: Jim Farkas 512 N. Salisbury Street, Office 640M Raleigh, NC 27604 For USPS: Jim Farkas 1612 Mail Service Center Raleigh, NC 27699-1612 Hand Delivery: Please reach out to me prior to hand delivering a submission to make sure that I (or someone else in my group) will be able to receive the submission. Do not leave the package in the foyer with the security guard. 1 NOTE: Hard copies should not be sent to a Regional Office. Doing so will delay the review process and the submission package may be lost while being sent from the Regional Office to me in the Central Office. Electronic copies should be uploaded at the following location: https://edocs.deq.nc.gov/Forms/SW-Supplemental-Upload Since these are really minor things, once I get the hard copies, anything else that may have changed from addressing these issues, and the documents are uploaded to Laserfiche (Provided everything is still in order), we can issue the permit without doing another formal review. The quickness of the next review for this project is dependent on reviewer availability. Let me know if you have any questions.
