This office has reviewed your Private Facilities permit application(s) and is authorizing the
building of the structure(s) subject to the following conditions:
The construction of the facility must be completed as described in the approved
application and within twelve (12) months from the date of this approval.
Lake Services must be notified upon completion of the structure(s) so the lake use
permitting process can be completed; a permit tag will be attached at final inspection (if
one is not already present on an existing structure).

Please place a copy of this letter in a watertight plastic bag and post it along the
shoreline where you are implementing your approved activity.

New, expanded or rebuilt facilities or mooring buoys must be situated or constructed in
size, dimension, or design such that an average-size (10 foot minimum) moored
watercraft will not interfere with access to other facilities and not obstruct ingress and
egress of watercraft.

Note: Permitees must immediately notify Duke and stop all work if historical or
archeological resource are discovered during the implementation of an authorized project.
Although DE-LS has reviewed and approved your Private Facilities Permit Application,
because not all applications are submitted with plans that are drawn to scale, the
applicant must ensure that all the requirements of Duke’s Shoreline Management
Guidelines are met, including but not limited to; (1) the structure described in the
Application does not extend beyond 1/3 the distance of the cove or one hundred and
twenty feet (including any moored watercraft); (2) facilities must be situated or
constructed in size, dimension, or design such that an average-size moored watercraft
will not interfere with access to other permitted facilities and not obstruct ingress and
egress of watercraft, and; (3) structure must not exceed the maximum allowed square
footage defined by the minimum shoreline requirements.

Please note: This approval does not approve electrical service on dock or piers. Lot owners should contact
their county Buildings and Codes office for electrical service requirements and/or restrictions. Some
counties may restrict submersible pumps in the lakes.
