ORC Matthew Smith was present during the inspection. A review of records, data, operations, maintenance, sampling, treatment components, and structures was completed. This review reflected noncompliance with the subject permit. The determined permit violation(s) and item(s) of concern are listed below. Inspection findings are detailed in the attached Water Compliance Inspection Report. Permit Violations 1. No records for field thermometer NIST-traceability, calibration, or verification using a NIST- traceable device were available. Part II, Section D. (6.) of the subject permit requires that the permittee retain all calibration records for at least three years. Please see the attached approved procedure for the analysis of temperature by the North Carolina Wastewater/Groundwater Laboratory Certification Branch for requirements related to the calibration, verification, and NIST-traceabili of temperature -measuring devices. 2. According to the provided records, the effluent flow meter was last calibrated June 30, 2021. Part II, Section D. (3.) of the subject permit requires flow measurement devices to be calibrated at a minimum of once per year. 3. Due to the lack of a properly maintained right of way to the outfall, upstream sampling has not been conducted at the location specified by the subject permit. Part I, Section A. (1.) of the subject permit designates the upstream sampling location as 20 feet upstream from the outfall. Part II, Section D. (1.) of the subject permit requires that all samples shall be taken at the monitoring points specified in the permit. The influent manual bar screen was severely corroded with portions of bars corroded away and/or missing, and therefore unable to adequately screen debris. Part II, Section C. (2.) of the subject permit requires that all facilities and systems of treatment and control (and related appurtenances) which are installed or used by the permittee shall be properly operated and maintained at all times. 5. The equalization basin audible and visual alarms were tested during the inspection and found to be nonfunctional. Part II, Section C. (2.) of the subject permit requires that all facilities and systems of treatment and control (and related appurtenances) which are installed or used by the permittee shall be properly operated and maintained at all times. Items of Concern 1. A copy of the current NPDES permit was not available on site during the inspection. 2. Theequalization basin structure had significant visible corrosion. 3. Duckweed was observed covering approximately 15% of the surface of the clarifier. 4. Some solids, duckweed, and other vegetation were observed partially blocking the clarifier overflow weirs. 5. When tested during the inspection, the generator appeared to operate properly for approximately 10 minutes, then malfunctioned and had to be manually restarted by the operator. 6. A right of way to the outfall had not been properly maintained. Please be aware that the Division may take enforcement action against any individual who fails to comply with the requirements of all applicable permits, regulations, and/or statutes. Pursuant to NC General Statute (NCGS) 143-215.6A, a civil penalty of not more than twenty-five thousand dollars ($25,000.00) may be assessed against any person who violates or fails to act in accordance with the terms, conditions, or requirements of any permit, or applicable regulations, issued pursuant to G.S. 143-215.1. Please reply, in writing, to this notice of violation within ten (10) business days after receipt. Your written response must address each violation listed above and provide your intended remedial actions, with estimated completion dates, for each violation. A review of your response will be considered along with any additional information provided. You will then be notified of any civil penalties that may be assessed regarding the violations. Section D: Summary of Finding/Comments (Attach additional sheets of narrative and checklists as necessary) On June 7, 2023, Division of Water Resources (DWR) Winston-Salem Regional Office (WSRO) staff Jesse Barnes conducted a routine compliance evaluation inspection at the Milton WWTP in reference to NPDES permit NC0087645. ORC Matthew Smith was present during the inspection. A review of records, data, operations, maintenance, sampling, treatment components, and structures was completed. This review reflected noncompliance with the subject permit. The following violations were noted: 1. No records for field thermometer calibration were available. Part II, Section D. (6.) of the subject permit requires that the permittee retain all calibration records for at least three years. Other observations: Page# Permit: NCO087645 Inspection Date: 06/07/2023 Owner -Facility: Milton WWTP Inspection Type: Compliance Evaluation NPDES permit NCO087645 became effective March 1, 2023 and will expire at midnight on May 31, 2027. The facility and its components were as described in the subject permit. The subject permit contains no special conditions. Access to the plant site was restricted to the general public by a locked fence. The inspector was granted access to all areas for inspection. Required records were available and appeared complete and current. Mr. Smith stated that records for at least the past three years have been stored at Pace Analytical in Kernersville, NC and were not on site during the inspection. The DMR, laboratory data, and chains of custody for April 2023 were reviewed and appeared to be complete with no errors found. All ORCs held the appropriate certification level at the time of inspection. An ORC visitation log was available and current. A copy of the current NPDES permit was not available on site. Field parameters of pH, DO, and temperature have been performed under field lab certification #633, held by Pace Analytical. All other parameters have been transported to and performed by Pace Analytical commercial lab certification #40. Field calibration records for pH and DO were reviewed and appeared current and complete. No records for field thermometer calibration were available, Mr. Smith stated that he utilizes the thermometer built into the DO meter for field temperatures. The facility has utilized pH, DO, and a sludge judge for process control parameters throughout the treatment process. The plant was generally clean with acceptable housekeeping during the inspection. Mr. Smith stated that influent grab samples are taken from the flow splitter box located just after the bar screen. The influent manual bar screen was severely corroded with several bars corroded away entirely, and therefore unable to adequately screen debris. The equalization basin was aerated with submerged forced air diffusers and appeared to be free of excessive grease, solids, and organic matter. The equalization basin audible and visual alarms were tested during the inspection and found to be nonfunctional. The high level alarm was manually triggered and displayed a prompt on the control panel, but the alarm light and sound did not commence. The equalization basin structure was highly corroded. The aeration basin appeared to be operating properly during the inspection and utilized submerged forced air diffusers. All diffusers appeared to be operational, and the basin was free of dead spots. Some duckweed was observed in the aeration basin, covering approximately 5% of the surface. The secondary clarifier did not contain a center well or drive unit, only a baffle and weir. The clarifier was free of black and odorous wastewater. The clarifier appeared to be free of excessive floating sludge. Mr. Smith stated that he maintains a sludge depth of approximately 1.5 feet in the clarifier, this was not measured during the inspection. Duckweed was observed covering approximately 15% of the surface of the clarifier. Some solids, duckweed, and other vegetation were observed partially blocking the clarifier overflow weirs. Due to the blockages and low flow during the inspection, the clarifier effluent wastewater was difficult to inspect and characterize. UV disinfection is utilized at the facility and the apparatus appeared to be operating properly during the inspection. Due to low flow, effluent leaving the UV apparatus was difficult to inspect and characterize. Enough backup bulbs were stored on site to replace every bulb in the unit. Page# Permit: NCO087645 Inspection Date: 06/07/2023 Owner -Facility: Milton WWTP Inspection Type: Compliance Evaluation Dual 3,000 gallon sludge holding tanks were being used to store excess sludge and are pumped and hauled as needed. Pump and haul records and/or receipts were not requested during the inspection. The tanks appeared to be storing sludge properly during the inspection, with adequate capacity available. An automatic generator capable of operating the entire facility was on site during the inspection and has been tested weekly under load with the primary power source interrupted. The generator is fueled by propane, the level of which is monitored and refilled by Thomas Oil Company. When tested during the inspection, the generator appeared to operate properly for approximately ten minutes, then malfunctioned and had to be manually restarted by the operator. The effluent flow meter has been used for reporting and appeared to be operating properly during the inspection. According to the provided records, the effluent flow meter was last calibrated June 30, 2021 by Clearwater Inc. Flow meters used for monitoring and reporting must be calibrated annually and those records retained for three years, according to the subject permit. The effluent pipe and outfall were not inspected due to inaccessibility caused by tall vegetation and safety concerns. A right of way to the outfall had not been properly maintained. A right of way to the outfall must be maintained for the purposes of inspection, maintenance, and sampling. Due to the lack of a properly maintained right of way to the outfall, upstream sampling has not been conducted at the location specified by the subject permit, which is 20 feet upstream from the outfall. Mr. Smith stated that downstream sampling has been conducted at the permit -specified location of Country Line Creek at Highway 57, but the location was not inspected due to time constraints.
