Division of Water Resources' Central and Regional staff has reviewed the application package
received November 21, 2022. However, additional information is required before the review may be
completed. Please address the items on the attached pages no later than the close of business on March 2,
2023.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
in Sections A through B, or failure to provide the additional information on or before the above requested
date may result in your application being returned as incomplete pursuant to 15A NCAC 02T .0107(e)(2).

A. Letter from Raleigh Regional Office:
1. The submitted application package for the Wrenn Road Wastewater Irrigation System included a
reference to a letter from Scott Vinson, with the NCDEQ Raleigh Regional Office, that was received
in June of 2022. In this letter, Mr. Vinson agreed that the limit for manganese (Mn) should be raised
to 0.057 mg/L for this facility. A copy of this letter was not included in the submitted application.
Please submit a copy of this letter.
B. Well Construction Record Form (GW-1):
1. Please submit a Well Construction Record Form (GW-1) for each of the new wells that have
been installed (PMW-1, PMW-2, PMW-3, PMW-4). Please include the location of these wells using
latitude and longitude coordinates.
