Parks Freeze 407 Cascade Dr High Point, NC 27265 NORTH CAROLINA Environmental Quality July 17, 2023 Subject: Compliance Evaluation Inspection 407 Cascade Dr, High Point, NC 27265 Single -Family Residence Domestic Wastewater Treatment System NPDES WW General Permit No. NCG550000 Certificate of Coverage No. NCG550149 Davidson County Dear Mr. Freeze: On June 9, 2023, Division of Water Resources (DWR) Winston-Salem Regional Office (WSRO) staff Jesse Barnes conducted a routine compliance evaluation inspection at the single-family residence wastewater treatment system located at 407 Cascade Drive, High Point, NC 27265, in reference to NPDES general permit NCG550000 and certificate of coverage NCG550149. Mr. B Parks Freeze, owner, was present during the inspection. A review of the subject permit, structures, treatment components, operations, and maintenance was completed. This review reflected noncompliance with the subject permit. Items of concern observed during the inspection are listed below. Items of Concern 1. Mr. Freeze stated that no effluent monitoring has been conducted, as required by Part I, Section C. (1.) of the subject permit. 2. The contact chamber well, below the chlorinator tubes, had been damaged by a fallen tree and was leaking onto the ground during the inspection. Contractors were on site during the inspection and planned to repair the damage. 3. The exact location of the outfall was unknown and not observed during the inspection. Please be aware that the Division may take enforcement action against any permittee who fails to comply with any requirement of their permit. Pursuant to NC General Statute (NCGS) 143-215.6A, a civil penalty of not more than twenty-five thousand dollars ($25,000.00) may be assessed against any person who violates or fails to act in accordance with the terms, conditions, or requirements of any permit issued pursuant to G.S. 143-215.1. The contact chamber well, below the chlorinator tubes, had been damaged by a fallen tree and was leaking onto the ground during the inspection. Contractors were on site during the inspection and planned to repair the damage. 3. The exact location of the outfall was unknown and not observed during the inspection. Mr. Freeze stated that he believes the outfall discharges to a pond located north/northwest of his property and that the outfall pipe may have been disturbed during construction of the pond and adjacent golf course. He has not been able to locate the outfall pipe since the area was disturbed. Other observations: Mr. Freeze stated that the septic tank has been pumped approximately annually, records were not requested. Two tablet chlorinator tubes were observed during the inspection, contained tablets, and appeared to be operating properly. Mr. Freeze stated that he applies approximately two tablets to each chlorinator tube monthly. According to the tablet manufacturer's label, the tablets in use were 90% chlorine tablets designed for recreational pool disinfection. Mr. Freeze was instructed to use domestic wastewater disinfection tablets with the active ingredient calcium hypochlorite.
