Everything looks good except for a few small issues: Please correct the following with SW6201005 (Phase I Minor Modification): - Supplement-EZ Form: o Cover Page:  Line 2 – Please use the project area (13.29 ac) instead of the total property area (19.42 ac). o Drainage Area Page:  Lines 5 & 6 – Please use the project area (578,912 sf) instead of the total property area (845,721 sf).  Line 18 – NOTE: Line 18 = Line 8 / Line 5 = 130938 sf / 578,912 sf = 22.6% ≠ 24%. Please revise as needed. Please correct the following with SW6220902 (Phase II Application): - Swale Design – Swales 4, 6, 7, 8, & 11 have a permanent matting. Please ensure that the design information in the Supplement-EZ Form reflects this (specifically, the Vallowable, Vactual, and Flow Depth). NOTE: The Vactual should not exceed the Vallowable (Vallowable can be increased based on the matting rating). - While not required, you may wish to explain (in the additional information section on the low-density page) that the 100 ft minimum length downstream of the curb outlet system is being provided partially by swale and partially by vegetated receiving area so the swale may be shorter than 100 ft long. Typically, either swales or vegetated receiving areas are used to meet this requirement so clarification may be useful to help avoid potential issues for the permittee in the future. Please provide the following: - 1x hard copy & 1x electronic copy of the revised pages of the Supplement-EZ Forms for the subject projects with the above comments addressed. Hard copies should be mailed to me at the following address: For FedEx/UPS: Jim Farkas 512 N. Salisbury Street, Office 640M Raleigh, NC 27604 For USPS: Jim Farkas 1612 Mail Service Center Raleigh, NC 27699-1612 Hand Delivery: 1 Please reach out to me prior to hand delivering a submission to make sure that I (or someone else in my group) will be able to receive the submission. Do not leave the package in the foyer with the security guard. NOTE: Hard copies should not be sent to a Regional Office. Doing so will delay the review process and the submission package may be lost while being sent from the Regional Office to me in the Central Office. Electronic copies should be uploaded at the following location: https://edocs.deq.nc.gov/Forms/SW-Supplemental-Upload I have drafted up the permits already so if you could email me the revised pages of these two forms (I can print out hard copies and upload the electronic files), we can issue the permit mod without a lengthy re-review (should only take a day or so). Sorry for the delay, Let me know if you have any questions.
