Division of Water Resources' Central and Regional staff has reviewed the application package
received on April 17, 2023. However, additional information is required before the review may be
completed. Please address the items on the attached pages no later than the close of business on August
10, 2023.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
in Sections A through O, or failure to provide the additional information on or before the above -requested
date may result in your application being returned as incomplete pursuant to 15A NCAC 02T .0107(e)(2).

The parcel number information is absent from Sections II, VI, and IX for the treatment facility,
open atmosphere structure, and the utilization fields. It is difficult to verify that the Permittee owns
all the land associated with the wastewater system according to the Dare County GIS website
(https://maps.darecountync.gov� because the parcel that contains the facility also contains several
condominium properties under separate ownership but the parcel map does not provide a
delineation of where these properties are. To verify that the treatment, storage, and disposal fields
are owned by the Permittee, please provide parcel information and deeded owner for each treatment
system component.
2. Section IX references the previous permit's Attachment B for information regarding the utilization
fields. However, this information does not include cover crop information. This section shall be
completed to include the parcel information and the cover crop.

agreement will not be valid.
2. Along with the Operational Agreement, the application form also requires the Articles of
Incorporation, Declarations, and By -Laws for legally formed Homeowners' Associations. In
accordance with the application and 02T .0115(c), this document shall be provided.
