To date, the Division's records show that the following civil penalties assessed
against your facility remain unpaid:

NC0042251 LV-2023-0022 $1,290.56
The total amount of unpaid penalties is $2,831.12. These penalties are now subject
to collection by the Attorney General's office (AGO), pursuant to NCGS 143-215.6A
(g).
