Mr. Jerry Bowman, owner, was present during the inspection. A review of the subject permit, structures, treatment components, operations, and maintenance was completed. This review reflected non- compliance with the subject permit. Items of concern observed during the inspection are listed below. Items of Concern 1. I Mr. Bowman stated that no effluent monitoring has been conducted, as required by the subject permit. Please be aware that the Division may take enforcement action against any permittee who fails to comply with any requirement of their permit. Pursuant to NC General Statute (NCGS) 143- 215.6A, a civil penalty of not more than twenty-five thousand dollars ($25,000.00) may be assessed against any person who violates or fails to act in accordance with the terms, conditions, or requirements of any permit issued pursuant to G.S. 143-215.1. Section D: Summary of Finding/Comments (Attach additional sheets of narrative and checklists as necessary) On May 30, 2023, Division of Water Resources staff Jesse Barnes conducted a routine compliance evaluation inspection at the single-family residence wastewater treatment system located at 1314 Canterbury Trail, Asheboro, NC 27205, in reference to NPDES general permit NCG550000 and certificate of coverage NCG550572. Mr. Jerry Bowman, owner, was present during the inspection. A review of the subject permit, structures, treatment components, operations, and maintenance was completed. This review reflected non-compliance with the subject permit. Mr. Bowman stated that no effluent monitoring has been conducted, as required by the subject permit. Mr. Bowman stated that the septic tank has been pumped approximately annually but was unsure of exact dates. Two tablet chlorinator tubes were observed during the inspection and appeared to be operating properly. Mr. Bowman was instructed to use domestic wastewater disinfection tablets with the active ingredient calcium hypochlorite in the chlorinator tubes. The effluent outfall pipe was observed during the inspection but there was no flow at the time. The effluent pipe was black and discharged into a ditch located approximately twenty feet from the receiving stream.
