Thank you. I’m just getting caught up on emails from the holiday break. I have revised the Application (part E3 &

E4) per your comments, below. However, your 2 comment asks to provide the updated pages of the Supplement
EZ-form, but only the Application-to-Construct (ATC) form was revised by these comments. Therefore, just let me
know: Did you only need the revised page from the ATC form, or do you need to see something revised on the
EZ-form?

I’ll get the hardcopies sent to you as soon as I hear from you.

Mr. Jones,

I was looking over the information you provided on 12/15/2022. Everything looks good except for a few small issues:

Please correct the following:
- Application, Part E3 – The river classification and index number for the Yadkin Pee-Dee River are not required.
Please revise.
- Application, Part E4 – Only the immediate downstream receiving surface water information is required. Please
revise.

Please provide the following:
- 2x hard copies & 1x electronic copy of the revised page of the Supplement-EZ Form. NOTE: New signatures on
this form are not required, we only need the updated pages.

Hard copies should be mailed to me at the following address:

Hand Delivery:

NOTE: Hard copies should not be sent to a Regional Office. Doing so will delay the review process and the
submission package may be lost while being sent from the Regional Office to me in the Central Office.

Since these are really minor things, once I get the hard copies, anything else that may have changed from addressing
these issues, and the documents are uploaded to Laserfiche (Provided everything is still in order), we can issue the
