This letter transmits a Notice of Violation and assessment of civil penalty in the amount of S354.31 (S250.00 civil penalty
+ $104.31 enforcement costs) against Clarke Utilities Inc.
This assessment is based upon the following facts: a review has been conducted of the Discharge Monitoring Report
(DMR) submitted by Clarkc Utilities Inc for the month of July 202'_. This review has shown the subject facility to be in
violation oft lie discharge limitations and/or monitoring requirements found in NPDES WW Pennit No. NC0073318. The
violations, which occurred in July 2022. are summarized in Attachment A to this letter.
