Description of projects area and activity: This verification authorizes the permanent discharge of fill material into 0.015 acre (95 linear feet) of stream channel, necessary to re -align a portion of stream channel to protect existing multi-familv residential infrastructure from an eroding stream bank. Any violation of the attached conditions or deviation from your submitted plans may subject the permittee to a stop work order, a restoration order, a Class I administrative penalty, and/or appropriate legal action. This verification will remain valid until the expiration date identified below unless the nationwide and/or regional general permit authorization is modified, suspended or revoked. If, prior to the expiration date identified below, the nationwide and/or regional general permit authorization is reissued and/or modified, this verification will remain valid until the expiration date identified below, provided it complies with all requirements of the modified nationwide permit. If the nationwide and/or regional general permit authorization expires or is suspended, revoked, or is modified, such that the activity would no longer comply with the terms and conditions of the nationwide permit, activities which have commenced (i.e., are under construction) or are under contract to commence in reliance upon the nationwide and/or regional general permit, will remain authorized provided the activity is completed within twelve months of the date of the nationwide and/or regional general permit's expiration, modification or revocation, unless discretionary authority has been exercised on a case -by -case basis to modify, suspend or revoke the authorization. Project Boundaries. The project will take place in an area of limited space. The project boundaries are shown on plan sheet 2. The limits of the project boundaries will be staked and flagged at the site by the ENGINEER prior to commencement of construction activities. 2. Equipment Storage and Materials Staging Areas. An area within the project boundary has been established for temporary storage of equipment and materials. This area is shown on plan sheet 3 and will be staked and flagged at the site by the ENGINEER prior to commencement of construction activities. No storage of equipment or materials shall take place in the road and parking areas. 3. Protection of Existing Building. The project work will take place very close to the existing building at 3939 A Valley Court. Before construction begins, suitable blanket type materials shall be temporarily attached to the south facing wall. The protection will extend along the entire width of the wall and to a height of 10 feet above ground. 4. Fall Protection Fences. The CONTRACTOR shall furnish and install a fall protection fence shall be placed along the southern embankment of the stream as shown on plan sheet 3. 5. Security Fencing. In order to secure the project site from the public, the CONTRACTOR shall furnish and install a fall protection fence beginning at the southeast corner of 3939 A Valley Court, extending to the sidewalk, making a 90 degree turn and follow the curb to a point 75 feet from the building, ending at a point along the southern stream bank. Signage shall state "Construction Area -Do Not Enter". The Location is shown on plan sheet 3. 6. Construction of Temporary Equipment access to stream. The stream channel will be accessed by a temporary path. The location of the temporary access corridor is shown on plan sheet 7. The ENGINEER shall stake the centerline of this corridor prior to construction. The maximum width of the corridor shall not exceed 15 feet. Heavy equipment to be used in the stream channel shall not deviate from this access corridor. If needed, a geotextile can be placed directly on the ground surface and a 6-inch thickness of crusher run placed over the geotextile to minimize damage to the access corridor during inclement weather. 7. Disposal of Waste Materials and Debris. All non -vegetative materials and debris shall be removed from the project site at the end of each day by the CONTRACTOR. 8. Excavated Materials. All soil, gravel and sand excavated to form the new stream channel shall be used as backfill in the existing stream channel. PROJECT CONSTRUCTION SEQUENCE • Remove flagged trees along the eastern streambank within the project boundary. All materials shall be mulched on -site. • Install all fall protection and security fencing. • Install the wall protection blankets. • Construct the temporary access corridor. • Install erosion control elements as shown on plan sheet 3. • Excavate the new trapezoidal channel as per the cross-section shown on plan sheet 7. Place the excavated material in the existing stream channel. • Excavate the wall fin and footing. Place the excavated material in the existing stream channel and between the footing excavation and the eastern stream bank. Should excessive water be encountered during excavation, it shall be pumped to a point where the natural channel is encountered below the limits of the footing excavation. • A separate CONTRACTOR will remove the existing sewer manhole and replace with a new manhole and piping. • Install concrete forms in the fin and footing. • Install rebar in the fin and footing. • Pour the concrete for the fin and footing. A concrete truck and boom shall be set up in the parking lot and the concrete boomed to the fin and footing. • Construct the retaining wall forms. • Install the water control measures behind the wall forms. • Install the wall rebar in the forms. • Pour the concrete for the retaining walls. • Remove the wall forms. • Excavate, haul and place engineered fill between the retaining wall and the existing embankment. • Seed and mulch the fill. • Remove all erosion control measures, fencing, and wall protection blankets. • Restore any area of the grounds and yards that were damaged during construction. PROJECT SPECIFICATIONS Division 1.0 Clearing and Grubbing 1.1. Scope The work consists of clearing and grubbing and disposal of trees, snags, logs, brush, stumps, shrubs, and rubbish from the designated areas. 1.2. Protection of existing vegetation Trees and other vegetation designated to remain undisturbed shall be protected from damage throughout the duration of the construction period. Any damages resulting from the contractor's operations or neglect shall be repaired by the contractor. Earthfill, stockpiling of materials, vehicular parking, and excessive foot or vehicular traffic shall not be allowed within the drip line of vegetation designated to remain in place. Vegetation damaged by any of these or similar actions shall be replaced with viable vegetation of the same species, similar condition, and like size unless otherwise approved by the contracting officer. Any cuts, skins, scrapes, or bruises to the bark of the vegetation shall be carefully trimmed and local nursery accepted procedures used to seal damaged bark. 1.3. Marking The limits of the area(s) to be cleared and grubbed will be marked by stakes, flags, tree markings, or other suitable methods by the ENGINEER. Trees to be left standing and uninjured will be designated by special markings placed on the tree limbs about 6 feet above the ground surface. 1.4. Clearing and grubbing All trees not marked for preservation and all snags, logs, brush, stumps, shrubs, rubbish, and similar materials shall be cleared from within the limits of the project in the existing stream. Unless otherwise specified, all stumps, roots, and root clusters that have a diameter of 1 inch or larger shall be grubbed out to a depth of at least 2 feet below subgrade in the area of the fin and footing of the concrete retaining wall and 1 foot below the ground surface at embankment sites and other designated areas. 1.5. Disposal All materials cleared and grubbed from the designated areas shall be hauled from the site and disposed of by the CONTRACTOR. The CONTRACTOR is responsible for complying with all local rules and regulations and the payment of any and all fees that may result from disposal at locations away from the project site. Division 2.0 Excavation 2.1 Designated Areas Excavation shall be limited to the area of the new stream channel and the retaining wall footing. No excavation shall take place outside the stream banks. All materials excavated during construction of the new stream channel shall be placed in the existing stream channel. After the channel is filled, additional excavated materials shall be placed in the area behind the retaining wall footing and spread to equal elevations. 2.2 Dewatering During excavation of the fin and footing, excess water shall be removed by pumping to facilitate construction of the forms, installation of the rebar and pouring of the concrete. The water shall be disposed of at a point in the existing stream below the newly constructed channel. 2.3 Foundation Bearing Pressure A bearing pressure of 2,500 psf. was assumed for the foundation. I� . � �11��C�1�1i .�aL•9i 3.1 Location and Materials A construction corridor has been established for the new channel as shown on Plan Sheet 7. A trapezoidal shaped channel shall be constructed within this corridor with dimension shown on Plan Sheet 7. The earth materials within the corridor are predominately sand and gravel. Excavated materials shall be handled as specified in Division 2.1. 3.2 Shape The new channel may include slight meanders within the corridor to mimic natural conditions. The slope of the bottom of the channel shall be positive in the downstream direction. 3.3 Channel Cutting In order to provide the correct grade of the bottom of the channel, excavation shall begin at the downstream end and move upstream. The upstream termination point shall join the existing stream at the same elevation. No potholes or pools shall be formed within the new channel. AGeosciencee orthwest PC t_ P.O. BOX 24487 Winston-Salem NC 27107 336.414.7011 Division 4.0 Footina Forms 4.1 Materials Following excavation of the fin and footings for the retaining wall, concrete forms will be constructed in the excavation. The materials to be used for the forms shall be untreated pine 2 x 4's, % inch plywood and wooden stakes that can be easily removed following curing of the concrete. 4.2 Disposal Upon removal, the forms shall be removed from the site and disposed of off -site in compliance with local regulations covering construction and demolition debris. Division 5.0 Retainina Wall Forms 5.1 Materials Materials for the retaining walls shall be new 2x4s and % inch plywood sheets or pre -fabricated metal forms. 5.2 Construction The wall forms shall be constructed above the concrete footing after it has been poured and cured. 5.3 Removal The retaining wall forms shall remain in -place for 48 hours following the concrete pour. Pre -fabricated metal forms shall be removed and placed on trailers for removal. Wooden forms shall be removed and disposed of in compliance with local regulations governing disposal of construction and demolition materials. Division 6.0 Enaineered Backfill 6.1 Selection and Source The CONTRACTOR shall provide a five -gallon bucket of the proposed engineered backfill to the ENGINEER for testing and approval. The source of the fill shall be designated. 6.2 Suitability Soils suitable for engineered fill shall be clean, free of organic materials, contain no hazardous materials, and have Unified Soil Classifications MIL, CL, SC, or SC. 6.3 Placement and Compaction Engineered fill shall be placed between the new concrete retaining wall and the existing stream bank adjacent to the building. Placement will utilize a track -hoe to remove the fill from dump trucks and gently dump the fill from the track -hoe bucket. Extreme care must be taken to prevent hitting the building and the new retaining wall with the bucket. The fill will be placed in 8-inch lifts and compacted with the track -hoe bucket and rammer. The ENGINEER shall approve placement of each additional lift after compaction. Division 7.0 Retaining Wall Drainage System 7.1 Description A horizontal drain shall be constructed behind the concrete retaining wall just above the footing prior to backfilling. The drain will entail a 4-inch diameter corrugated plastic pipe with 1/2-inch holes along the top encased in a 1 foot by 1 foot envelope of washed #57 stone. The washed stone envelope will be shrouded by a geotextile filter fabric to prevent intrusion of back fill. The lower end of the drain will emerge from the extension of the wall into natural ground and discharge to the channel. 7.2 Materials The 4-inch diameter single wall corrugated plastic pipe shall comply with ASTM F2986-12 or equivalent. The geotextile filter shall be a non -woven fabric such as Hanes Geo Components Terra Tex SD or equivalent. The washed #57 stone shall be equal to ASTM C 33 stone. 7.3 Installation Installation of the drainage system shall take place after the concrete retaining wall has been poured and all the wall forms removed. The geotextile shall be laid on the base of the footing on the inside of the retaining wall (between the wall and the near bank). A three-inch layer of #57 stone shall be placed on the geotextile extending from the back of the wall to the edge of the footing. The corrugated plastic pipe shall be laid on the layer of #57 stone with the holes pointing upward along the top of the pipe. Additional #57 stone shall be placed on the pipe so that a 1-foot envelope of stone surrounds the corrugated plastic pipe. The geotextile shall then be wrapped around the stone and folded over the top. Division 8.0 Rebar 8.1 Materials Rebar shall conform to the table located on the right hand side of this sheet: Division 9.0 Concrete 9.1 Materials Concrete shall comply with the requirements in the table located on the right hand side of this sheet: Concrete to be used in the fin and footings shall utilize 3000 psi hydraulic cement. Concrete to be used in the retaining wall shall be 3000 psi. The concrete for the footings shall conform to a consistency class of S4. The concrete for the retaining wall shall conform to a consistency class of S3. Concrete compaction shall use either a poker or beam vibration and/or tamping. Division 10.0 Seeding and Mulching 10.1 Area for Seeding The fill area behind the retaining wall and any portion of the existing yard disturbed by construction activities shall be permanently seeded. 10.2 Materials Seed shall be Serica Lespedeza applied at a rate of 15 Ibs/acre with a broadcast spreader. Fertilizer shall be Scott's Turf Builder Triple Action Starter Fertilizer with a N-P-K ratio of 24-25-4 or equal. The fertilizer shall be applied at a rate of 100 pounds per acre. Erosion control blankets conforming to American Excelsior or equivalent shall be placed over the seeded area and held in place by steel staples. 10.3 Soil Preparation The soil shall be tilled to a depth of 4-inches to facilitate seed germination. All stones greater than one inch, sticks, roots and other extraneous materials that surface during soil preparation shall be removed. Division 11.0 Erosion Control 11.1 Temporary Check Dam Prior to any clearing or excavation in the stream channel, a temporary rock check dam shall be installed at the location shown on the plans. The check dam shall extend across the width of the entire channel from high bank to high bank. The check dam shall have a bottom width two feet and a height of one foot. The check dam shall consist of 6-inch diameter rip -rap. 11.2. Silt Fence A silt fence shall be installed on the lower side of the temporary stream access corridor as shown on the plans. The silt fence materials and installation methods shall comply with the latest North Carolina Department of Energy, Mineral and Land Resources Erosion and Sediment Control Manual. Division 12.0 Site Restoration 12.1 Requirements • Following final inspection and approval of all work by the ENGINEER, the CONTRACTOR shall remove all construction debris, trash, fall protection fencing, wall protection blankets and any other temporary materials. The temporary access corridor to the stream shall be restored by removing any stone and geotextile. Any damage and erosion to the corridor shall be restored by seeding and mulching in compliance with Division 10 above. Any erosion control devices or materials placed in the stream channel shall be removed.
