The inspection was conducted to determine if the system is being operated and maintained in compliance with General NPDES Permit No. NCG520000. Mr. Jason Deck (owner) was present during the inspection. At the time of the inspection, the facility was not in operation. Mr. Deck indicated that the facility has not been in operation since prior to 2021, but that they would be in operation in the next couple months. The settling basin appeared full, but Mr. Deck stated they will remove four feet of sediment prior to their next dredging event. The settling basin is cleaned out approximately one to two times per year. Mr. Deck also indicated that signage is in place upstream and downstream of the dredging operation to warn canoeists and other recreational water users of the operation. Upstream and downstream turbidity sampling will be conducted when the facility is in operation moving forward, as the facility has not been in operation since before 2021. Sampling for turbidity was a requirement starting with the effective date of the Certificate of Coverage under the 2021 permit renewal for the NCG52 General Permit. Mr. Deck indicated that average effluent flow is 2.3 MGD, and that he will send turbidity sampling results and a flow estimate next time the facility is in operation. 23/06/01 C DocuSign Envelope ID: CC30ED54-15C4-4CAA-BA5F-272B72853DFC Hines Road Pit Compliance Evaluation Owner - Facility: Inspection Type: Permit: NCG520119 Inspection Date: 06/01/2023 3 Page# Permit Yes No NA NE (If the present permit expires in 6 months or less). Has the permittee submitted a new application? Is the facility as described in the permit? # Are there any special conditions for the permit? Is access to the plant site restricted to the general public? Is the inspector granted access to all areas for inspection? Comment: Operations & Maintenance Yes No NA NE Is the plant generally clean with acceptable housekeeping? Does the facility analyze process control parameters, for ex: MLSS, MCRT, Settleable Solids, pH, DO, Sludge Judge, and other that are applicable? Comment: Effluent Pipe Yes No NA NE Is right of way to the outfall properly maintained? Are the receiving water free of foam other than trace amounts and other debris? If effluent (diffuser pipes are required) are they operating properly? Comment: At the time of the inspection, the facility had not been in operation for two years, so there was no discharge. When the facility is in operation, the settling pond enters a concrete ditch which then discharges to surface water. Effluent Sampling Yes No NA NE Is composite sampling flow proportional? Is sample collected below all treatment units? Is proper volume collected? Is the tubing clean? # Is proper temperature set for sample storage (kept at less than or equal to 6.0 degrees Celsius)? Is the facility sampling performed as required by the permit (frequency, sampling type representative)? Comment: The facility has not been in operation since before 2021. Mr. Deck indicated that average estimated flow at the effluent of the facility is approximately 2.3 MGD when in operation.
