Please use the latest version of that form (available here: https://deq.nc.gov/about/divisions/energy-mineral-and-land-
resources/stormwater/stormwater-program/stormwater-design).
A lot of the information asked for in the old version of the form (what was provided) is similar to what is asked for in the
newest version of that form, but there are some differences in the information asked for so we do not accept the old
version of that form anymore.

Good Afternoon,
We received your application for a new post-construction stormwater permit on 7/27/2022. At this time, the submission
is not administratively complete and cannot be accepted for review in its current condition. Please provide the following
submittal items within 30 calendar days (by close of business on 8/31/2022) so that we may accept the project and
begin the review process, otherwise, the project will be returned as administratively incomplete.
1x hard copy of the complete Application Form. This item is required per 15A NCAC 02H .1042(2)(a). The
majority of the Application Form does not appear to be completed, Section III, 1, Section VIII, and Section X (plus
some checked boxes in Section II) are the only sections that appear to have been completed. Please complete
the remaining sections of the Application.
1x original signed & sealed hard copy of the latest version of the Supplement-EZ Form. This item is required per
Section V & VI, 3 of the Application. The provided hard copy is outdated (we no longer use that form) Please use
the Supplement-EZ Form available at the following address: https://deq.nc.gov/about/divisions/energy-mineral-
and-land-resources/stormwater/stormwater-program/stormwater-design
1x application review fee check in the amount of $505. This item is required per 15A NCAC 02H .1042(2). We did
not receive a review fee check in the submittal package (It is noted that this item was included on the letter of
transmittal as being part of the submission).
2x signed & sealed hard copies of a plan sheet showing the following:
Drainage area to the proposed SCM. This item is required per Section VI, 8o of the Application and 15A
NCAC 02H .1042(2)(g)(iv). Please include the drainage area to the SCM in the main set of plans.
Wetland delineation or a note on the plans stating that there are no wetlands located on-site (Required
per Section VI, 8m of the Application and 15A NCAC 02H .1042(2)(g)(iii)). Please also list the name of the
individual that made this determination in a note on the plans. This item must be provided in the main
set of plans.

Maintenance access & easements. These items are required per Section VI, 8n of the Application,
General MDCS 8 & 9, and 15A NCAC 02H .1042(2)(g)(vi). Please ensure that the SCMs are accessible for
maintenance and that the access/easement is shown on the plans.
NOTE: This is an administrative completeness review, not a detailed engineering review, there may be other issues that
may be brough up during the engineering review.
If these items are received within the stipulated timeframe, we can accept the project and begin the review process.
Once accepted, reviews under the standard permitting process can take up to 90 calendar days per statute (one of our
two reviewer positions is currently vacant).
