However, additional information is required before the review may be completed. Please address the items on the attached pages no later than the close of business on May 1, 2023. Please be aware that you are responsible for meeting all requirements set forth in North Carolina rules and regulations. Any oversights that occurred in the review of the subject application package are still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items in Sections A through C, or failure to provide the additional information on or before the above requested date may result in your application being returned as incomplete pursuant to 15A NCAC 02T .0107(e)(2). Flow Information: 1. During review of the flow information listed in Section III of Form: WWIS-R 02-21 to be less than half of the previously permitted value. I first wanted to double check that the current average flow is indeed 1,500 gallons per day. I also wanted to double check on flow contributions. The previous permit made special note of the allocated flow from both the grocery store/multi-purpose retail and drugstore. Please provide the flow from each source. B. Operator in Responsible Charge (ORC) Information: 1. Per 15A NCAC 02T .0507(b), the permittee shall designate a certified operator in responsible charge (ORC) and back-up ORC. 15A NCAC 02G .0201(2) further specifies that the Back-up ORC shall "...possesses a valid certificate of the type of the system and no more than one grade less than the grade of the system, with the exception of no Back-up ORC is required for systems whose minimum visitation requirements are twice per year... ". Our records show the current Back-up ORC to be Bradley T. Flynt. The facility is classified as both a Surface Irrigation (SI) and Wastewater-1 (WW-1) system. Our records do not show whether or not Mr. Flynt has an SI Certification. Does Mr. Flynt have an SI Certification? C. Open -Atmosphere Structures: 1. Section VI of Form: WWIS-R 02-21 requires all Open -Atmospheric Structures to be listed. The only structure listed is the 550,000 gallon storage pond. The previous permit issuance also listed two wetland cells as existing Open -Atmospheric Structures. Please confirm whether these treatment systems are still in use.
