3a. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application:
The property is the water supply well site for this Canterbury Estates Subdivision. The site is wooded with mixed hardwoods and
pines. the property has a stream that flows to Crooked Creek and then to the Eno River and then to the Neuse River. There is an
existing 10' X 10' well house that has a concrete slab floor and a stick build structure. The well house was constructed in 1979 and is
now just inside the Neuse River's 100' buffer.

4. Proposed Activity
Provide a detailed description of the proposed activity including its purpose and include the type of equipment to be used:
Recent changes to the state's drinking water limits requires the Community well be upgraded to iron and M The proposed work
consists of expanding 8 feet to the front of the building to install two manganese filters consisting of 2 each 36" diameter tanks and
associated piping. Additional to this project, a 12' X 24' concrete slab to support two 5,000 gallon polyethylene ground storage tanks.
Waste backwash water will be stored in the 5,000 gallon tanks until it is hauled to a wastewater treatment plant. The site is flat and
minimal grading will be required for the building slab. The site for the storage tank slab will require fill material to level it up to
proposed grade. Grading and trenching will be performed by backhoe. No sanitary wastes will be generated for this project.

Comments:
The site currently used as a water supply well and requires an upgrade to treat the water for iron and manganese and radium. We
need to expand the building by adding an 8' X 10' extension onto the existing building to house the new filters. Since the filter
backwash water may contain radium, we will install two storage tanks to contain the backwash water until it can be pumped and
hauled to a wastewater treatment plant of which Aqua North Carolina owns and operates. Land disturbance will be minimal and will
be completed in less than two months.
D. Impact Justification and Mitigation
1. Avoidance and Minimization
la. Specifically describe measures taken to avoid or minimize the proposed impacts in designing the project.
The building is minimally expanded a sufficient to contain the new filters.
The storage tank slab is sized just enough to support the tanks and seismic restraints holding the tanks.
Land disturbance will be minimal. Just enough to install the slabs and connecting piping.
1b. Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques
Silt fence will be installed on the down slope side of the site to catch nay sediment will flow that way.
A construction entrance will be constructed to minimize the tacking of mud off the site.
Construction will take less than 2 months and the site will be stabilized as soon as construction is completed.

Additional Comments:
We are under a compliance order due to the new water quality limits that requires the installation of the new filters. The amount of
radium in the water has been increasing and is expected to exceed the state limits before the end of the year. Since a water supply
well is an allowable activity in the buffer and the well has been in use since 1979, we respectfully requesting a quick review and
approval of this project.
