Prevention Plan and a qualitative and an analytical monitoring report to the Winston-Salem
regional office by February 22, 2023, or enforcement action may be taken.

This inspection was conducted in response to a complaint about concrete poured down a slope. The operator stated the
concrete was poured to stabilize the slope. The facility did not have the SPPP, qualitative or analytical monitoring reports.
