Dear Mr. Dellinger and Mr. Phillips,
Enclosed please find a copy of the Compliance Evaluation Inspection Report for the
inspection we conducted at the Winston-Salem Plant 6 on September 27, 2022. Please note that
in the stormwater inspection report summary we ask that you submit employee training records,
facility inspections, and a qualitative monitoring report by January 6, 2023, and a final incident
report at the completion of the clean up for the fire incident to the Winston-Salem regional
office.

Inspection Summary:
The Stormwater Pollution Prevention Plan (SPPP) has been developed, but documentation of employee training and facility
inspections was missing. This inspection was conducted in response to a fire that cut off the power to the oil water
separator which overflowed through the outfalls. The facility has a "first flush" box that normally collects the fine particulates
for the cullet storage bins, but this water was turbid at the time of inspection due to water still draining from the firefighting
activities in the basement where the oil water separator is located. The facility used absorbent booms down the outlet and
in the channel to clean up the discharged oil. The cullet storage area still needed further cleaning to prevent the fine
particulates and residual oil from discharging through the outfalls. Additional measures like covering the cullet storage bins
and inserting filters into the storm drain inlets in the cullet storage area is recommended to reduce turbidity in the "first flush"
outfall. The five-day incident report has been received by the Winston-Salem regional office and a final report needs to be
submitted once the incident has been fully resolved. The missing documentation for employee training, facility inspections,
and a qualitative monitoring report must be submitted to the Winston-Salem regional office by January 6, 2023. If you have
