If the answer to question 4 is "YES" list the affected wells individually with constituent(s) and concentration(s) exceeding standards in the space provided below: MW-8 was sampled on March 09, 2023 and the result for total nitrate nitrogen was 11 mg/L which was above the permit limit of 10 mg/L; however, the well is an upgradient background well located in the vicinity of a septic system and drain field. The result may be influenced by the septic system and does not appear to be related to the facility. 5 For the constituents identified in question 4 above, have standards been exceeded previously for the same Yes X No constituent(s) in the same well(s) in the last two years?
