However, additional information is required before the review may be completed. Please address the items on the attached pages no later than the close of business on March 22, 2023. Please be aware that you are responsible for meeting all requirements set forth in North Carolina rules and regulations. Any oversights that occurred in the review of the subject application package are still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items in Sections A through T, or failure to provide the additional information on or before the above -requested date may result in your application being returned as incomplete pursuant to 15A NCAC 02T .0107(e)(2). Per the County GIS, S & J Villari Livestock, LLC is the listed landowner. Please revise the landowner's name on Page 3. 5. Please have the signature authority for this permit application package sign the Permittee's Certification at the bottom of Page 3. 6. Please submit a separate Form: LOA 06-16 for Field 1 with RSJ Properties, LLC information listed under the Landowner's Certification. Why is there no setback radius delineated for the second habitable residence located at 813 Penny Branch Rd? The most recent aerial imagery shows this house is still present. The habitable residences located at 790 Penny Branch Rd and 392 Bruce Costin Rd appear to be within 400 feet of the residual application areas as well. Please either: a. Reduce the residual application area of Fields 1 and 2 to accommodate the habitable residence setback radii of 400 feet; or b. Obtain a signed, notarized, and recorded setback waiver from the current property owners located at 790 Penny Branch Rd, 813 Penny Branch Rd, and 392 Bruce Costin Rd. If previous setback waiver(s) have been executed, please provide them instead. A blank setback waiver with S & J Villari Livestock LLC's information has been provided for your convenience; or c. A combination of the options above that either reduces the residual application area or obtains a setback waiver for the habitable residences in question. Please provide an updated vicinity/setback map that shows any changes in acreage to the residual application area, the habitable residences in question, and their full 400-foot setback radii or a reduced setback that corresponds to a signed, notarized, and recorded setback waiver. There appears to be a dirt/gravel driveway in the western portion of Field 2. Please reduce this residual application area accordingly and update the setback/vicinity map to reflect the latest site conditions. 3. Please revise "Area B" to "Area 1" and "Area A" to "Area 2" to match the existing permit. 4. There appears to have been some parcel consolidation since the vicinity/setback map was last updated.
