Dear Mr. Montebello::
The purpose of this letter is to provide a background narrative to accompany a Pump and Haul
permit application for the above reference parcel. This parcel has had an Orange Co subsurface
permit which has now been revoked for several reasons, and now it seems the only option is to
pursue a General Discharge permit. The revocation of the County permit is because an
additional bedroom Accessory Dwelling Unit was built upon the lot. It was permitted for
construction by the Town of Chapel Hill, but not by the County Health Department. According
to the State & County OnSite Rules this additional bedroom must count as two bedrooms worth
of flow. This causes the parcel to lose its "repair area" exemption and there is no system repair
area available on the parcel. This leaves the property owner somewhat of a limbo because the
County says they can't issue a P&H permit on a system that they can't permit. So .... in the
meanwhile of putting together the documentation for a Stream Discharge Permit application, (the
RRO has visited the site and has determine it is most likelihood a valid candidate), and for during
the time of its review and permit issuance, the Property Owner needs a P&H permit, thus
providing a legitimate means of wastewater disposal during this interim period to avoid the
eviction of current residents.

15A NCAC 02T .0200 — WASTEWATER PUMP AND HAUL SYSTEMS
Division of Water Resources INSTRUCTIONS FOR FORM P&H 01-16 & SUPPORTING DOCUMENTATION
Documents shall be prepared in accordance with 15A NCAC 02T .0100, 15A NCAC 02T .0200, and Division policies. Failure to submit
all required items will necessitate additional processing and review time. For more information, visit the Wastewater Branch's
website or contact the Regional Office serving your county.
General —This permit application applies to all pump and haul activities of wastewater under the authority of the Division on Water
Resources. Permit coverage is required for the system producing the wastewater. This permit does not apply to the transport of
animal waste from an animal waste management systems, not the transport of wastewater residuals or biosolids.
NOTE: Pump and haul permits are not acceptable long term domestic wastewater treatment alternatives and shall only be issued
in cases of environmental emergencies, nuisance conditions, health problems, or for unavoidable delays in construction of system
previously permitted by the Division.
The Appficant shall submit one original and one copy of the ap plication and supporting documentation.
A. Pre -submittal Contact & Cover Letter

B. Application Fee (Domestic Wastewater Only)
® Submit a check made payable to: North Carolina Department of Environmental Quality (NCDEQ)
➢ The appropriate application fee is: $810.00 for flows < 10,000 GPD and $1,310.00 for flows > 10,000 GPD
C. Wastewater Pump and Haul (FORM: P&H 01-16) Application:
® Submit the completed and appropriately executed Wastewater Pump and Haul (FORM: P&H 01-16) application. Any
unauthorized content changes to this form shall result in the application package being returned. If necessary for clarity or
due to space restrictions, attachments to the application may be made, as long as the attachments are numbered to
correspond to the section and item to which they refer
D. Receiving Facility Letter:
® Submit a letter from the owner/authority of the receiving wastewater treatment facility, as required by 15A NCAC 02T .0204
(b), stating that the pumped and hauled wastewater will be accepted and specifying the volume and type of wastewater that
will be accepted. The letter should be signed by an authorized signing official in accordance with 15A NCAC 02T .0106 b
E. Contract Hauler Letter:
® Submit a letter from the hauler stating their capability and willingness to perform the pump and haul activity
® Letter shall include: Owner/Company Name, Contact Information, & Hauling Volume of Vehicle(s)
F. Certifications:
® The Applicant's Certification on Page 4 of the application shall be signed in accordance with 15A NCAC 02T .0106(b). Per 15A
NCAC 02T .0106(c), an alternate person may be designated as the signing official if a delegation letter is provided from a
person who meets the criteria in 15A NCAC 02T .0106(b)
® The Professional Engineer's Certification on Page 5 of the application shall be signed, sealed and dated by a North Carolina
licensed Professional Engineer (if required)
G. Plans and Specifications (Required for activities that result in the Practice of Engineering per G.S. 89C-3):
® Submit two sets of plans and specifications (signed, sealed and dated by a N.C. Licensed Professional Engineer)
® Plans must include:
➢ A general location map
➢ Diagram of the components associated with the pump and haul activity (drains, piping, tanks, pumps, etc)
➢ Tank Details that indicate high water alarm
➢ Other items as required

Industrial Pump and Haul Activities
Per 15A NCAC 02T .0203 A 2ti, Industrial pump and haul activities are deemed permitted provided the following criteria is met:
➢ The facility notifies the appropriate Division regional office in writing advising of the type of operation, type and quantity of
wastewater generated, location, and the receiving wastewater treatment facility. A letter from the facility accepting the
wastewater (type and quantity) specifically agreeing to accept wastewater from the applicant shall be included.
➢ The wastewater does not contain any human waste.
➢ The waste is collected and discharged into a sewer or treatment system designed and permitted to accept the type of
wastewater being pumped and hauled.
➢ The pump and haul activity is not to alleviate a failing wastewater system.
➢ The Division regional office concurs in writing that the activity meets the criteria in this Rule.

Hello all,
McFarland Septic currently has a valid Authorization to Discharge for domestic septage with the
City of Durham. Therefore; based on correspondence, I hereby grant permission for the
acceptance of the 100% domestic waste generated (approximately 350 gallons per day) from 510
Gooseneck Road, Chapel Hill, NC for the length of the assumed and soon to be issued Pump and
Haul from the Raleigh Regional Office.
• There have been instances where the site(s) had to be closed on short notice due to blockages and plant performance.
Haulers continue to be advised to find alternate dump sites to the City of Durham.
• Be reminded that the City of Durham site hours are Monday through Friday from 800 am — 300 pm. There are no hours on
the weekend. To discharge outside of the established hours is a safety issue that the City of Durham does not entertain.
For the reasons above, I state implicitly that it is the responsibility of the project to find alternate
dump locations if / when the City cannot accept waste during these or any other periods. I require
that additional dump locations be added to the pump and haul as a condition of granting
permission for discharge with the City of Durham. For the bulleted items above and basic
logistics, the City of Durham should not be the lone dump site. The City of Raleigh, OWASA and
some other municipalities have Septage Programs.
Be advised that the City of Durham reserves the right to halt any discharge found detrimental to
any portion of its collections system or treatment facilities at any time.

Here is what I have started so far for an application. I have to sort out a few more things before I can actually submit it. A letter
from the receiving WWTF is required to show the state where its going, their ability and willingness to accept it, etc.
Refer to item "D" of the instructions, and Item "IV" of the application for necessary info for me to fill out.

McFarland Septic responsibilities:
1. Pump holding tanks of contents (up to 2500 gallons) at 510 Gooseneck Road, Chapel Hill NC
27514, deliver and dispose of contents at an approved septage or sewer receiving facility.
2. Keep a record of the dates pumped from the facility.
3. Notify the NCDWR immediately of any problems with the system by calling

SUBJECT: REQUEST FOR PUMP AND HAUL AUTHORIZATION
Dear Mr. Balltzglier:
Recently you contacted the Mason Farm Wastewater Treatment Plant about discharging septage
wastewater at the Wastewater Treatment Facility of Orange Water and Sewer Authority (OWASA)
collected through a pump and haul process needed while a septic system is being repaired. OWASA
approves your request. Please follow the terms and conditions below.
The terms and conditions applicable to the use of the septage receiving facility at the Mason Farm WWTP
are:
1. Only wastewater from 510 Gooseneck Rd, Chapel Hill, NC will be permitted for discharge into this
facility for the next 12 months from the acceptance of this authorization letter. If additional days are
needed beyond the timeframe of acceptance, please contact Travis Rich with OWASA.
2. A routine sample per tanker is to be collected along with a completed Septage Receiving Record form
which identifies the source and volume of the wastewater must be provided to the Mason Farm WWTP
Staff at the time of each wastewater discharge.
3. The charges for handling this wastewater will be billed on a monthly basis. These charges are
$182.63/thousand gallons plus a $30 administration fee for each individual discharge at the MFWWTP.
Failure to pay the charges billed for this service will result in revocation of the authorization to
discharge into the OWASA wastewater facilities.
4. Routine use of the septage facility at the Mason Farm Plant is during normal staffing hours at the facility
(Monday -Friday 8:00 A.M.4:00 P.M.). Any other arrangement must be made in advance.
