Please see attached for the groundwater monitoring report associated with Injection Well Permit
#W10700012 and the Tank Farm Remedial Action Plan. This report includes the sampling and reporting
requirements of the above referenced permit as well as the reporting requirements of the Tank Farm Remedial
Action Plan. Enclosed are a) piezometer level monitoring field sheets, b) the operation and maintenance
reports and c) semi-annual lab analysis results of withdrawal wells.
Please note that analytical results for Total Phosphorus and Fluoride parameters were analyzed outside of
the required holding times. This has been noted on the lab analysis summary page. The data was qualified by
comparing results of the current monitoring period to historical averages for each monitoring location.

QUARTERLY OPERATIONAL AND MAINTENANCE REPORT FOR THE INJECTION AND
WITHDRAWAL WATER WELLS
February 2023 — April 2023
The Tank Farm Remediation System consists of a series of seven- (7) injection water wells located on the
south side of the Tank Farm Area and a series of fourteen (14) withdrawal water wells located on the north
side of the Tank Farm Area.
Infection Water Well System
The injection water well system is operated with mine depressurization water (DPW).
Daily operational data is collected by the Shipping Department and reported to the Environmental Affairs
Department. The total monthly and quarterly volumes of water injected as well as the month end water level
in the Water Level Control Tank are:
