Staff review of the plans and specifications has determined that the project, as proposed, will comply with the Stormwater Regulations set forth in Title 15A NCAC 2H.1000. We are forwarding Permit No. SW8 960329 dated July 9, 1996, to Camp Lejuene Marine Corps Base. This permit shall be effective from the date of issuance until July 9, 2006, and shall be subject to the conditions and limitations as specified therein. Please pay special attention to the Operation and Maintenance requirements in this permit. Failure to establish an adequate system for operation and maintenance of the stormwater management system will result in future compliance problems. If any parts, requirements, or limitations contained in this permit are unacceptable, you have the right to request an ajudicatory hearing upon written request within thirty (30) days following receipt of this permit. This request must be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings, P.O. Drawer 27447, Raleigh, NC 27611-7447. Unless such demands are made this permit shall be final and binding. The tract will be limited to the amount of built -upon area indicated on page 3 of this permit, and per approved plans. 5. The permittee is responsible for verifying that the proposed built -upon area does not exceed the allowable built -upon area. 6. The following items will require a modification to the permit: a. Any revision to the approved plans, regardless of size. b. Project name change. C. Transfer of ownership. d. Redesign or addition to the approved amount of built -upon area. e. Further subdivision of the project area. f. Filling in, altering, or piping of any vegetative conveyance shown on the approved plan. In addition, the Director may determine that other revisions to the project should require a modification to the permit. 7. The Director may notify the permittee when the permitted site does not meet one or more of the minimum requirements of the permit. Within the time frame specified in the notice, the permittee shall submit a written time schedule to the Director for modifying the site to meet minimum requirements. The permittee shall provide copies of revised plans and certification in writing to the Director that the changes have been made. IL SCHEDULE OF COMPLIANCE The permittee will comply with the following schedule for construction and maintenance of the stormwater management system: a. The stormwater management system shall be constructed in it's entirety, vegetated and operational for it's intended use prior to the construction of any built -upon surface except roads. b. During construction, erosion shall be kept to a minimum and any eroded areas of the system will be repaired immediately. C. All connections into the stormwater system from future areas/outparcels shall be made such that short-circuiting of the system does not occur. The,permittee shall at all times provide the operation and maintenance necessary to assure the permitted stormwater system functions at optimum efficiency. The approved Operation and Maintenance Plan must be followed in it's entirety and maintenance must occur at the scheduled intervals including, but not limited to: a. Semiannual scheduled inspections (every 6 months). b. Sediment removal. C. Mowing and revegetation of side slopes. d. Immediate repair of eroded areas. e. Maintenance of side slopes in accordance with approved plans and specifications. f. Debris removal and unclogging of outlet structure, orifice device and catch basins and piping. g. Access to the outlet structure must be available at all times. 3. Records. of maintenance activities must be kept and made available upon request to authorized personnel of DEM. The records will indicate the date, activity, name of person performing the work and what actions were taken. 4. This permit shall become voidable unless the facilities are constructed in accordance with the conditions of this permit, the approved plans and specifications, and.Qther supporting data. 5. Upon completion of construction, prior to issuance of a Certificate of Occupancy, and prior to operation of this permitted facility, a certification must be received from an appropriate designer for the system installed certifying that the permitted facility has been installed in accordance with this permit, the approved plans and specifications, and other supporting documentation. Any deviations from the approved plans and specifications must be noted on the Certification. 6. A copy of the approved plans and specifications shall be maintained on file by the Permittee for a minimum of ten years from the date of the completion of construction. III. GENERAL CONDITIONS 1. This permit is not transferable. In the event there is a desire for the facilities to change ownership, or there is a name change of the Permittee, a formal permit request must be submitted to the Division of Environmental Management accompanied by an application fee, documentation from the parties involved, and other supporting materials as may be appropriate. The approval of this request will be considered on its merits and may or may not be approved. 2. Failure to abide by the conditions and limitations contained in this permit may subject the Permittee to enforcement action by the Division of Environmental Management, in accordance with North Carolina General Statute 143-215.6(a) to 143-215.6(c). , 3. The issuance of this permit does not preclude the Permittee from complying with any and all statutes, rules, regulations, or ordinances which may be imposed by other government agencies (local, state, and federal) which have jurisdiction. r. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions, the Permittee shall take immediate corrective action, including those as may be required by this Division, such as the construction of additional or replacement stormwater management systems. 5. The permittee grants Regional Office Staff permission to enter the property for the purpose of inspecting all components of the permitted stormwater management facility. 6. The permit may be modified, revoked and reissued or terminated for cause. The filing of a request for a permit modification, revocation and reissuance or termination does not stay any permit condition. Permit issued this the 9th day of July, 1996. We are providing these sheets and the requested computations for your review. The following is a response to each of your comments: 1. The drainage areas are delineated on the plans and included in the calculations. 2. The drainage area for each of the ponds has been added to the plans. 3. The plan sheets and calculations have been signed and sealed. 4. The outlet structures are labeled on the plans. 5. The 1" storage level is shown on the pond details. 6. The permanent pool and design pool contours are shown on the plans. 7. As we discussed, the weepholes have hardware cloth coverings and #57 stone filter to prevent clogging. The grates for the inlets have been changed to prevent clogging. The detail for this grate is attached. The forebay weir has been raised to the permanent pool elevation. 9. The parking lot entrances have been revised to insure that the impervious surfaces drain to the ponds. 10. A note has been added to each pond detail to require that slopes be 3:1, or flatter. 11. The pond data has been separated on the application form. 12. The calculations for the drawdown holes are provided. 13. The attached calculations show the correct surface areas. 14. The SA/DA calculations have been provided for each pond, in accordance with the table you provided. We hope that these revised plans and additional calculations are sufficient for your approval, so that we may obtain the required permit. This is critical, as the client has already begun the bidding process and is anxious to proceed with this project. A preliminary review of that information has determined that the application is not complete. The following information is needed to continue the stormwater review: 1. The pond's volume is required to be calculated for the drainage basin to the pond, which includes the proposed impervious area, and all other drainage (both impervious and grassed) within the basin that cannot be routed around the pond. ✓2. Clearly delineate each pond's drainage area on the plans. / 3. Provide sealed copies of the design plans. 0%4. Label the outlet structure location on the plans. L/5. Draw and label the 1" storage elevation on the pond detail. �6. Draw and label the permanent pool and design pool contours on the plans. This Office must be able to verify the surface areas you report. c/7. Provide a trash guard for the 3/4" weepholes and the overflow from the pond. 1/ 8. I cannot find where in the calculations you arrived at the 3/4" size, nor can I find where the orifice was showm to'draw down the pond within the specified 2-5 day period. The calculations pertaining to 10 year and 100 year storm event routing are not required by this Office, only those requirements as listed in NCAC 2H.1000. 13. Is the "Max Surf Area" noted in the calculations the surface area provided at the 1" design storm elevation? 14. The State -has minimum surface areas that must be met based on the percent imperviousness of the drainage area, the TSS removal rate, and the depth of the pond. You have not provided information indicating compliance with this requirement. Attached is the SA/DA chart for your use. The project consists of two separate lots to be extended radially outward with associated lighting fixtures and landscaping. Stormwater ponds will be provided to detain the first inch of runoff from the proposed impervious areas. The total proposed impervious area is approximately 1.7 acres. Site Description The Naval Hospital currently has two separate lots - staff and patient - which are separated by 'B' Street (the entrance to the hospital) and 'D' Street. Behind the staff lot, there is a grass field and small wooded area (mostly large pines) which will be removed. Behind the patient lot is a soccer field and a small wooded area of which a portion will be cleared. The proposed extensions will not tie to existing drainage system but will have detention ponds to handle stormwater runoff. These ponds will be used as sediment basins during construction. Soils From boring logs at the time of the original Hospital construction, the native soils near the surface are predominately fine sands, with some clays and silts. Soils at depths of 6 to 10 feet contain clayey materials. Based on this limited data, we estimate that these native soils might be type SC or ML soils. Groundwater was measured at a depth of 8 feet in some of the borings. Storm Drainage Design The storm drainage system is designed for the 10-year storm. Run-off is calculated using the Rational Method and the Kirpich equation. Storm Drainage piping is sized to flow just full, without a surcharge in the system. The system calculations are attached. Stormwater Detention Calculations The two stormwater ponds for this project are designed to collect the first inch of runoff and detain this volume of water for three to five days. After the three to five day "drawdown" time, the water level in these ponds will return to the normal pool elevation. The ponds will collect the runoff from the proposed development only. Both ponds will be wet ponds with a normal K:\CIV\39419-06\W_P\RP7\STORMNARDOC 2 pool elevation of approximately three feet. Flood storage is provided above this normal pool elevation. The post -development run-off hydrograph was computed using small -watershed hydrograph methodology, developed by H. Rooney Malcolm, P.E., PhD, of North Carolina State University. The 6-hour, 10-year design hydrograph represents the pond inflow. Analysis of the stage -storage -discharge relationship for each of the ponds was performed using modified storage -indication routing methodology developed by Dr. Malcolm. This methodology develops a stage -storage function to describe the geometry of the pond. The relationship between the inflow hydrograph, storage volume and outflow rate was analyzed for a variety of pond and outlet configurations. The ponds were designed to pass the 100-yr post -development storm without over -topping the dam. See the attached calculations for a summary of the computations for each of the permanent ponds. The proposed ponds are to be used as temporary sediment basins for erosion control. Upon site stabilization, the filter stone will be cleaned, and utilized to prevent clogging of the drawdown holes.
