Dear Mr. Elia:
On July 14, 2016, the Wilmington Regional Office received a request to revise the approved plans for
Stortrwater Management Permit Number SW8 141106. The revisions include the addition of retaining
walls to achieve 3:1 bank slopes, and the modification of a perimeter grassed swale to direct stormwater
runoff from the cross culvert under Holden Beach Road to the existing ditch at the rear of the property.
Also, the proposed perimeter swale has been revised from a V-swale to a trapezoidal swale. There was no
additional built -upon area constructed. It has been determined that a formal permit modification is not
required for the proposed changes. We are forwarding you an approved copy of the revised plans for your
files. Please replace the old approved plan sheets with the new ones.
Please be aware that all terms and conditions of the permit issued on November 25, 2014, remain in
full force and effect, except as amended herein. Please also understand that the approval of this
revision to the approved plans or to the permit for the subject State Stormwater project is done on a
case -by -case basis. Any other changes to this project must be approved through this Office prior to
construction. The issuance of this plan revision does not preclude the permittee from complying with
all other applicable statutes, rules, regulations or ordinances which may have jurisdiction over the
proposed activity, and obtaining a permit or approval prior to construction.
Please be reminded that it is your responsibility as the permittee of record to submit a completed and
signed Name/Ownership Change Form to the Division of Energy, Mineral and Land Resources 60 days
prior to any of the ownership change events listed in Section 3.1 of the permit. These forms are available

II. PLAN REVISION INFORMATION

Retaining walls have been added to the site to achieve 3:1 bank slopes. A grassed swale has been added to
the rear of the property to direct existing runoff from the NCDOT culvert under Hwy 130 to the ex. wetland
III. SUBMITTAL REQUIREMENTS
Only complete application packages will be accepted and reviewed by the Division of Energy, Mineral and
Land Resources (DEMLR). A complete package includes all of the items listed below. The complete
application package should be submitted to the DEMLR Office that issued the permit.

Pursuant to a notice of violation by the Division of Land Quality, the Civil Site Plan has been revised to
add retaining walls to the site. A grassed swale has been added to the rear of the property to direct the
stormwater runoff from the cross culvert under Holden Beach Road to the existing ditch at the rear of the
property. The proposed perimeter swale has been revised from a v-swale to a rectangular swale to reduce
the velocity. The swale has been sized to carry the outflow from the 18" RCP under Holden Beach Road
flowing full.
Enclosed please find the following information submitted for your review and approval:
1) One (1) set of design drawings, Sheets C-1---C-4, dated 8-1-14, last revised 5-12-16.

Arnie,
It looks like the main changes are that you have added a retaining wall on one side, and extended a bypass
Swale around the other corner and down the back property line. Since these drainage issues are under County
jurisdiction and they do not directly affect the stormwater system, we do not need for you to do anything else
at this time. If you prefer, you can submit a Plan Revision just to clarify the site plans.
I have put our correspondence in the permit file for record keeping purposes.

The retaining wall along the rear of the property will end into the portion of the bank that is protected by rip
rap. The wall is proposed to lessen the slope of the grassed bank behind the rear parking lot to 3:1 max. The rip
rap extends from the emergency overflow weir out of the sand filter to the existing ditch.

Steve.
Revised plans were approved for this project September 2015. Since that time, the plans have been revised to
meet the maximum slope requirements of Land Quality.
Land Quality just approved the attached plans. Revisions also included adding a grassed swale alone the rear
of the property to provide a direct discharge to the existing ditch. The Swale was added to eliminate the
discharge to the rear adjacent property (which existed prior to construction).
The attached are provided for your review and approval.
