Attached is a copy of additional information, for the.subject project, that has been
submitted by Todd Shearin of Syn4gro. Generally, the additional information was
required because of necessary modifications that were determined during the site. visit;
h6w6vp'r,- Todd's transmittal letter. indicates that -some revised pages have been submitted
because of inconsistencies with landowner names that, he has corrected.

In reference to the site review that was conducted with'you on July 22, 2004, I• am sending you
the revised" maps with the increased buffers you requested. I also changed the. seasonal high
water table depth listed on the. -soil description -,'sheet f6rNC=CH-12-1 to 27 inches and had Eric
Thompson initial the changes. In addition, as.I. was corlipiling the maps, I. noticed, several
inconsistencies with the landowner names on the entire set of maps and'associated forms and
tables. Therefore,.I corrected these and am sendiilg-those along with the other corrections. '
Please forward copies to the appropriate persons.,
