Dear Mr. Mann:
Effective August 1, 2013 the State Stormwater program has been transferred from the Division of Water
Quality (DWQ)to the Division of Energy,Mineral and Land Resources (DEMLR). All previous
references to DWQ will remain in older stormwater permits issued prior to August 1, 2013 until they are
modified.
On March 15,2022,the DEMLR received your application for a Utility Line Project to be covered under
the State Stormwater General Permit Number SWG040000. In accordance with your application,we are
forwarding herewith the subject Certificate of Coverage Number SWG040152, along with a copy of the
General Permit,for the construction of a linear utility line project with associated incidental built-upon
area. The General Permit is issued pursuant to the requirements of North Carolina General Statute 143-
215.1 and Title 15A NCAC 2H .1000,the stormwater management rules.
Please take notice that this Certificate of Coverage is not transferable except by action to and approval by
DEMLR. The DEMLR may require modification or revocation and reissuance of the Certificate of
Coverage.
This permit does not affect the legal requirement to obtain other permits which may be required by the
Division of Energy,Mineral and Land Resources,the Division of Water Resources, Coastal Area
Management Act, or any other Federal, State, or Local agency, law,rule, or ordinance.

LINEAR UTILITY LINE PROJECT AND
ASSOCIATED INCIDENTAL BUILT-UPON AREA
In compliance with the provisions of North Carolina General Statute 143-215.1 , as amended,
other lawful standards and regulations promulgated and adopted by the North Carolina
Environmental Management Commission, and 15A NCAC 2H.1000, the Stormwater rules,

is hereby authorized to construct 21 ,345 square feet of built-upon area incidental to the
proposed utility line installation located at
NC 762 Pressure Zone Water Transmission Main and Finished Water Pump Station
3522 New Salem Road, Monroe, Union County
and to discharge stormwater to receiving waters designated as Richardson Creek, Bull
Branch, Jacks Branch, and Austin Branch, all class C waters in the Yadkin Pee-Dee River
Basin, in accordance with the provisions of the General Permit for a Linear Utility Line,
No.SWG040000, and the approved stormwater management plans and specifications, and
other supporting data as attached and on file with and approved by the Division of Energy,
Mineral and Land Resources and considered a part of this permit for the subject project.
This Certificate of Coverage shall become effective June 8, 2023.
This Certificate of Coverage shall remain in effect for the duration of the General Permit
SWG040000.
