GENERAL NOTES:

prior to conxriencenent of work.
3. 'As of January I, 1956, anyone digging with mechanized
equipment in highway right-of-way, private utility easements,
or public spaces will be required by North Carolina Law to
give notice of the proposed excavation to all the existing
area utilities at least 2 working days before starting to dig.
The utility owner is required to locate its facilities in the area
of the proposed excavation. Utility members of ULOCO can
be contacted at 1-800-632-4949.
4. Contractor is responsible for contacting any Cowl utility
companies that are not members of ULOCO.
5. Contractor is responsible for any damages to any existing
utilities.
6. Traffic stripes, arrows, and signs shall be per the North
Carolina Department of Transportation standards.

NOTE:
Contractor shall insure that he and all his workers (contractors,
subcontractors and other site personnel) do not damage construction
stakes or other measures used in laying out the project whether by
the Engineer or another entity including those under the employ of the
contractor. Contractor shall cause his workers to observe and verify
to the fullest extant practicable that stakes, other measures and grades
are, or appear to be, accurate and correct, and immediately, but prior
to any associated work, notify the staking entity of any potential conflict,
error or question regarding the work. The Engineer shall not be responsible
if all of the above measures are not strictly adhered to. The Engineer shall
not be responsible where due to damage, his staking can not be verified.
For critical or costly components of the work, where the contractor believes
that staking may not be preserved, the contractor shall recipest the staking
entity to place staking in an area where its preservation can be insured.

GENERAL NOTES:
I. Topographic information from actual survey
by Parke 4 Associates, Inc. April 2006.
2. Contractor shall verify location of all underground utilities
prior to commencement of work.
3. 'As of January I, 19m 86, anyone digging with echanized
eWipment in highway right-of-way, private utility easements,
or public spaces will be regpired by North Carolina Law to
give notice of the proposed excavation to all the existing
area utilities at least 2 working days before starting to dig.
The utility owner is recipired to locate its facilities in the area
of the proposed excavation. Utility members of ULOCO can
be contacted at 1-800-632-4949.
4. Contractor is responsible for contacting any local utility
companies that are not members of ULOCO.
5. Contractor is responsible for any damages to array existing
utilities.
6. Traffic stripes, arrows, and signs shall be per the North
Carolina Department of Transportation standards.
7. Handicap parking markings and signs shall be per the North
Carolina State Building Code - Accessibility Code Manual,
B. Contractor shall remove or relocate all existing structures
and utilities recipired prior to site construction.

11. The existing and proposed sidewalk shall be free of crocks, breaks

EROSION AND SEDIMENTATION CONTROL NOTES:
I. Sediment control devices and structures shall be installed,
maintained and amended as needed to provide effective
control of accelerated sedimentation until the contributing
watersheds are stabilized.
2. Construction Sequence:
A. Notify Owner, City of Jacksonville and Engineer prior to

om) B. Install stabilized construction entrances at all points of access.
C. Clearing or grading operations shall begin at low points, points of
outfoll release or at other locations where sediment control
devices (sediment traps, silt fences, diversions, stormwater
treatment areas, ect.) shall be installed before or simultaneously
d with the disturbance of any area draining to the device.
D. Relocate existing sewage gravity main.
E. Rough grade site. Install storm drainage pipe and
catch basins.
F. Install sewer service.
G. Install water service.
W. Rough grade drives and parking.
I. Install curb and gutter and CABC.
J. Construct building.
K. Fine grade drive and parking. Place CABC over
compacted subgrode. Install asphalt surface course.
L. Complete fine grading of landscaped areas in parking
lots and Ground building.
M. Maintain erosion control measures as needed to assure
full f nctionability.
N. Temporary or permanent seed, fertilize, and mulch all
disturbed areas within 15 days of completing any phase
of the grading work. If temporary seeding is used to
temporary stabilize an area, upon final grading of the job
the permanent seed, fertilizer and mulch shall be completed.
3. Seeding Specifications:
Apply time and fertilizer and work into 4 - 6 inches of soil. Seed
mixture shall be distributed uniformly and covered with a clean
straw mulch. All mulch shall be crimped or asphalt tacked to help
hold in place. In addition, geotextile fabric shall be used in areas
indicated. Material and application rates are as followed:

4. Contractor is responsible for maintaining all erosion control
measures and shall check and anend measures as required
after every rainfall event to prevent accelerated erosion
from taking place on this site.
S. Once areas have been stabilized, and accepted by the owner,
contractor is to remove temporary erosion control measures,
regrade, grass, fertilize, 4 mulch to restobilize these areas
will not be accepted until final permanent grass is established
with a complete cover and thriving and all areas are stabilized.
If areas do not stabilize, contractor shall repeat the above
until the areas is stabilized.
