Subject: Class A Residual Spill into Surface Water (Incident 202300872)
Dear Mr. Thomas,
uja
May 30, 2023
This letter is in regard to the conversation we had on May 26'', 2023 regarding incident
number 202300872 concerning Novozymes' Class A residuals that spilled from an
overturned tanker truck into the surface water of a nearby pond on May 26t°, 2023.
On May 261^, 2023 at 8:00 a.m. a tanker truck containing Class A residuals was parked
in a field at the land application site waiting to pull into the unloading area to offload the
residuals into a wagon for land application. While parked the truck overturned and re-
siduals leaked from the top of the tanker. The residuals ran across the ground and en-
tered the surface water of a nearby pond. The location of the spill is shown in Figure 1
at the red "X".

The residual application operators immediately responded to the incident. They re-
moved the driver from the truck and ensured the area was safe. The operators began to
pump out the remaining residuals from the tanker into a wagon to stop the residuals
from leaking onto the ground. Once the tanker was emptied, the operators pumped out
as much of the residuals from the pond as possible into a wagon.
Immediate investigations revealed that the tanker truck had parked on an underground
septic tank which collapsed due to the weight of the truck. When the septic tank col-
lapsed it caused the rear wheels of the tanker truck to fall into the septic tank which re-
sulted in the truck rolling over onto its side. It is estimated that 2500 gallons of residuals
leaked from the tanker truck onto the ground. The root cause for this incident was that
the landowner for this property was not aware of the underground septic tank so the op-
erators thatflag the application site were not aware of the hazard.
Please do not hesitate to call or email me should you have any questions regarding this
incident.
Best regards,
