The Reserve Lot 23, Ser fora, Hyco Lake
Dear Mr. Barker,
On July 23, 2014, the Division of Water Resources (Division) received an application, dated July 22;
2014, requesting a 401 Water Quality Certification from the Division for the subject project. The
Division has determined that "the application is incomplete, and cannot be processed at this time.
The application is on -hold until the following items are addressed:.)
1. According to ownership information on the Person County GIS website, the current owner
of the lot is Bobby and Lori Rose. If Legacy Building Co. LLC is the current owner, please
provide information that shows that ownership, transfer has occurred.
2. When the applicant.doesn -ot have title to the property, documentation must be provided
that the applicant has been authorized by the owner to apply for a certification. This can
be accomplished with a signed agent authorization form from the applicant and owner. An.
example can be found online:

3. The application proposes impacts along 800 feet of shoreline. These impacts may not be
covered under a General Permit 198200030 from'-the US`Army Corps of Engineers (USACE).
It may be necessary that you apply fora Nationwide 13 from the USACE and send in a pre -
construction notification form to the USACE and DWR. This form can be found at

check with Eric Alsmeyer with the USACE at eric .c.alsmever(cDusace.armv.mil or (919) 554-
