Dear Mr. Edwards:
The Division of Water Quality's NPDES Unit has reviewed your request for an Authorization to
Construct at the Dunn's Mobile Home Park WWTP. The request consists of design documents
received May 241h and revisions received October 3rd and 161h. The Division finds the proposed
improvements and design to be satisfactory and hereby grants authorization for the following:
• construction and installation of:
• a 3,560 gallon circular steel equalization tank;
• a 5,500 gallon circular steel aeration basin; and
• all related structures, equipment, piping, controls, and other appurtenances, as described in
the proposed design documents; and
• decommissioning of the existing polishing sand filter.
Upon completion of these improvements and submittal of the Engineer's Certification (see below),
the approved design capacity of the treatment facilities shall remain at 0.010 MGD.
This Authorization to Construct is issued in accordance with Part III, Paragraph A of NPDES
Permit No. NCO038300 issued November 19, 1996, and shall be subject to revocation unless the
wastewater treatment facilities are constructed in accordance with the conditions and limitations
specified in that permit.
Special Provisions
This Authorization is conditioned upon your meeting the following requirements:
1. Relocate the feed line or the overflow fine or both at the new aeration basin, in order to
minitnize short-circuiting of raw wastewater through the basin.

2. Modify the new aeration basin feed line, as necessary, to eliminate the possibility of its
siphoning water from the aeration basin back to the equalization basin. Options include
but are not limited to adding a check valve to the line or positioning the end of the feed
line above the top elevation of the basin.
3. Adjust the level switches in the equalization tank to provide a full 2,500 gallons capacity
(25% of average design flow) in the tank.
4. Evaluate the performance of the equalization system and, if necessary, modify the design
or operation to achieve the desired performance. The average design flow to the plant is
7 gpm, and the actual flow has been averaging only 2-3 gpm. However, the proposed
feed pump is rated at 14 gpm and has the potential to increase rather than decrease peak
flows through the plant.
These actions do not necessarily require prior approval of new design documents by the Division;
simply identify any substantial changes in the current design in the engineer's certification (see
below). However, any major revisions should be submitted for review.
In the event that the facilities fail to perform satisfactorily, including the creation of nuisance
conditions, the Permittee shall take immediate corrective action, including those as may be required
by this Division, such as the construction of additional or replacement wastewater treatment or
disposal facilities.
Notification Requirements
The Permittee shall notify the Raleigh Regional Office, telephone number (919) 571-4700, at least
forty-eight (48) hours in advance of operation of the installed facilities, so that an in -place inspection
can be made. Such notification to the regional supervisor shall be made during the normal office
hours of 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State Holidays.
Engineer's Certification
Upon completion of construction and prior to operation of the modified facility, the Pernnittee shall
provide the Division with a certification from a professional engineer certifying that the permitted
facility has been installed in accordance with the NPDES Permit, this Authorization to Construct,
and the approved plans and specifications. If the completed improvements differ substantially from
the approved design, the differences shall be noted in the Certification and documented
appropriately. Mail the Certification to the NPDES Unit, 1617 Mail Service Center, Raleigh, North

Certified Operator
The Permittee shall employ a certified wastewater treatment plant operator to be in responsible
charge (ORC) of the wastewater treatment facilities. The operator must hold a certificate of the type
and grade at least equivalent to or greater than the classification assigned to the Nastewater
treatment facilities by the Certification Commission. The Permittee must also employ a certified
back-up operator of the appropriate type and grade to comply with the conditions of Title 15A,
Chapter 8G, .0200. The ORC of the facility must visit each Class I facility at least weekly and each
Class II, III, and IV facility at least daily, excluding weekends and holidays; and must properly
manage and document daily operation and maintenance of the facility and comply with all other
conditions of Title 15A, Chapter 8G, .0200.

Additional Provisions
The Operational Agreement between the Pertrtittee and the Environmental Management
Commission is incorporated herein by reference and is a condition of this Permit. Noncompliance
with the terms of the Operational Agreement shall subject the Permittee to all sanctions provided by
G. S. 143-215.6 for violation of or failure to act in accordance with the terms and conditions of this
Permit.
Failure to abide by the requirements contained in this Authorization to Construct may subject the
Permittee to an enforcement action by the Division of Water Quality in accordance with North
Carolina General Statute 143-215.6A to 143-215.6C.
The issuance of this Authorization to Construct does not preclude the Permittee from complying
with any and all statutes, rules, regulations, or ordinances which may be imposed by other
government agencies (local, state, and federal) which have jurisdiction.
The Permittee shall maintain a copy of the approved plans and specifications on file for the life of
the facility. One (1) copy of the approved design documents is enclosed for your records.

Mr. Edwards -
Ted Cashon mentioned that you have not received the Authorization to Construct we issued November
17 for improvements at your treatment plant. We mailed both Ted's and your copies at the same time, so
I'm not cure why only one was delivered. In any event, I apologize for the inconvenience and am enclosing a
copy of the ATC for your use.
Ted also mentioned that (1) ATC did not show the correct facility name and (2) your permit does not
list the tablet dechlorinator and waste storage tank as parts of the treatment system. I will look into
these and get back to you within the next two weeks.

III. EVALUATION AND RECOMMENDATIONS
I have reviewed the submittal information for this project. The submittal indicates that
tanks will be added for aeration and flow equalization and that the existing sandfilter
will be eliminated. I am aware that the NPDES Unit has had several conversations with
the consultant regarding the proposed design. I have received a copy of some revisions
to the original proposal, but have not received any information nor heard of the
resolution of a question regarding the capacity of the pump to be used for equalization.
It appeared that the pump was much too large to function satisfactorily for flow
equalization. Please provide the RRO with a copy of any additional information
received, etc.
Upon resolution of the issue regarding the flow equalization pump and upon
satisfactory review of the project by the WQ NPDES Unit, the WQ RRO does not
object to the issuance of this A to C:
